摘　　要：目的分析冠心病患者住院费用的影响因素。方法基于山西省某三级综合医院2014—2020年4188例冠心病患者病案首页数据,采用Bootstrap面板分位数回归对住院费用的影响因素进行分析。结果药费、材料费和实际住院天数对住院费用各个分位点上均产生正向影响,自费比例则表现出相反结果。患者入院时病情在低分位数点上与住院费用关系为正,但在中高分位数点上关系为负且显著。年龄和是否手术对高分位上的住院费用有显著正向影响。结论不同影响因素对冠心病患者不同水平住院费用的作用差异显著,应通过探索医疗诊断相关分组(DRGs)支付和关口前移、加强此类疾病的预防等途径来减轻患者经济负担。Objective To analyze the influencing factors of hospitalization expenses for patients with coronary heart disease.Methods Based on the data of first page of the medical record of 4188 patients with coronary heart disease those were admitted to a tertiary general hospital in Shanxi province from 2014 to 2020,we analyzed the factors affecting the hospitalization expenses by using Bootstrap panel quantile regression.Results The cost of medicine,materials,and the hospitalization days had a positive impact on each quantile of hospitalization expenses,while the self-pay ratio showed the opposite result.At the low quantile,there was a positive relationship between the patient's condition and hospitalization expenses,while the relationship was negative and significant at the middle and high quantile.Age and surgery had a significant positive impact on hospitalization costs at the high quantile.Conclusions There are significant differences in the effects of influencing factors on patients with coronary heart disease at different expenses levels.It is necessary to explore medical diagnosis-related groupings(DRGs)payment,and moving the border forward,and strengthen the prevention of such diseases,to reduce the economic burden of patients.
