超声对宫外孕破裂与卵巢黄体破裂的鉴别诊断价值

摘　　要：目的分析超声对宫外孕破裂与卵巢黄体破裂的鉴别诊断价值。方法回顾性选取100例妇科急腹症患者的临床资料,根据疾病类型进行分组,分别为宫外孕破裂组(50例)和卵巢黄体破裂组(50例),全部患者均实施彩色多普勒超声诊断。分析两组影像学特征及各项指标。结果宫外孕破裂组血管阻力指数、脉搏指数高于卵巢黄体破裂组,动脉收缩期最大流速低于卵巢黄体破裂组;宫外孕破裂组极低阻力型频谱、无张力期血流频谱、舒张期血流频谱、血流表现点状及条状的患者占比高于卵巢黄体破裂组,高阻力型频谱、血流表现半环状/环状的患者占比低于卵巢黄体破裂组,肿块直径小于卵巢黄体破裂组,差异具有统计学意义(P<0.05)。结论在鉴别宫外孕破裂与卵巢黄体破裂时,彩色多普勒超声能够提供患者血流量特征及血流动力学参数等信息,有助于判断患者疾病类型。Objective To analyze the value of ultrasound in differential diagnosis of ectopic pregnancy rupture and ovarian corpus luteum rupture.Methods The clinical data of 100 patients with gynecological acute abdomen were retrospectively selected.According to the disease types,they were divided into the ectopic pregnancy rupture group(50 cases)and the ovarian corpus luteum rupture group(50 cases).All patients were diagnosed by color doppler ultrasound.Imaging features and indexes of the two groups were analyzed.Results The levels of resistance index and pulse index in the ectopic pregnancy rupture group were higher than those in the ovarian corpus luteum rupture,and the peak systolic velocity level was lower than that in the ovarian corpus luteum rupture;in the ectopic pregnancy rupture group,the proportion of patients with very low resistance spectrum,non-tension phase flow spectrum,diastolic phase flow spectrum,and blood flow manifestations of point and strip were higher than those in the ovarian corpus luteum rupture group;the proportion of patients with high resistance type spectrum,blood flow manifestations of semi-ring/ring were lower than those in the ovarian corpus luteum rupture group,and the mass diameter was lower than that in the ovarian corpus luteum rupture group,the difference was statistically significant(P<0.05).Conclusion Color Doppler ultrasound can provide patients with blood flow characteristics and hemodynamic parameters in the identification of ectopic pregnancy rupture and luteal rupture of the ovary,which is helpful to evaluate the disease type of the patients.
