摘  要:目的利用MRI评估正常成人视神经结构,测量视神经直径和视交叉(OC)的宽度与高度,建立正常参考值标准。方法回顾性选取153例健康成人,按照性别和年龄(18~39岁、40~59岁以及60岁以上)对被试进行分组,在快速三维容积式插入法屏气检查(3D-VIBE)序列测量球后4 mm、8 mm、管段、颅段、OC前7 mm处视神经直径,以及OC宽度、高度、大脑横径。在Dixon序列测量球后4 mm、8 mm处视神经直径和视神经鞘直径。结果3D-VIBE序列测得球后4 mm处视神经直径为(4.83±0.49)mm,球后8 mm处视神经直径为(4.24±0.47)mm,管段直径为(3.66±0.40)mm,颅段直径为(4.42±0.46)mm,OC前7 mm处直径为(4.43±0.47)mm,OC宽度为(13.73±1.28)mm,OC高度为(2.33±0.31)mm。Dixon序列测得球后4 mm处视神经直径为(2.68±0.35)mm,视神经鞘直径为(5.34±0.61)mm;球后8 mm处视神经直径为(2.29±0.36)mm,视神经鞘直径为(4.64±0.59)mm。结论Dixon序列可清楚显示视神经蛛网膜下腔间隙,测量视神经及视神经鞘结构参数较为准确,3D-VIBE序列因其良好的组织分辨率以及三维重建能力,可用于OC测量。Objective To evaluate normal adult optic nerve structure by MRI,measure the diameter of the optic nerve and the width and height of the optic chiasm(OC),and establish a standard of normal reference values.Methods The research retrospectively included 153 healthy adults and subjects were grouped by sex and age(18-39 years,40-59 years,and over 60 years).The three-dimensional volumetric interpolated breath-hold examination(3D-VIBE)sequence was performed to measure the optic nerve diameters posterior to the globe at 4 mm,8 mm,canal segment,cranial segment,and 7 mm anterior to the OC,as well as OC width,height,and the brain transverse diameter.The Dixon sequence was performed to measure the optic nerve and the optic nerve sheath diameters at 4 mm and 8 mm posterior to the globe.Results The 3D-VIBE sequence measured the optic nerve diameter of(4.83±0.49)mm at 4 mm posterior to the globe,the diameter of the optic nerve at 8 mm posterior to the globe was(4.24±0.47)mm,the diameter of the canal segment was(3.66±0.40)mm,the diameter of the cranial segment was(4.42±0.46)mm,the diameter at 7 mm anterior OC was(4.43±0.47)mm,OC width was(13.73±1.28)mm,OC height was(2.33±0.31)mm.Measured in the Dixon sequence,the diameters of the optic nerve and optic nerve sheath were(2.68±0.35)mm and(5.34±0.61)mm at the 4 mm posterior to the globe,(2.29±0.36)mm and(4.64±0.59)mm at the 8 mm posterior to the globe,respectively.Conclusion The Dixon sequence can clearly show the subarachnoid space of the optic nerve.It is more accurate in measuring structural parameters of the optic nerve and optic nerve sheath.In contrast,OC measurements can use the 3D-VIBE sequence due to its good tissue resolution and three-dimensional reconstruction ability.
