多层螺旋CT对肾周炎活体肾筋膜向上附着的解剖研究

摘　　要：目的探讨多层螺旋CT(MSCT)显示肾筋膜的向上附着及通连情况。方法选取输尿管结石患者450例,行MSCT扫描,在图像工作站进行图像后处理及多平面重建(MPR)观察肾筋膜的显示情况、肾筋膜向上的附着情况及肾周间隙的上方通连关系。结果双侧肾筋膜上缘前层与后层融合并附着于膈下腹膜。右侧肾筋膜前层向上附着于胆囊颈或后方肝下缘,后层附着于膈下腹膜。左侧肾筋膜前层向上附着于胰腺筋膜、脾脏下缘腹膜或左膈下内后方筋膜,后层上缘与膈下筋膜融合。肾周间隙上缘开放者,右侧与肝裸区通连,左侧与膈下间隙通连。结论肾筋膜的上缘附着点位置多样,MSCT及MPR图像能较好地观察肾筋膜向上的附着情况以及肾周间隙向上的通连关系。Objective To explore the superior attachment and communication of the renal fascia by multi spiral computed tomography(MSCT).Methods MSCT scanning was performed in 450 cases with ureteral calculi,and then the multiplanar reconstruction(MPR)images of perirenal space and its anatomical communications were obtained on a postprocessing workstation.Results The anterior and posterior layers of the renal fascia fused together at the upper pole of the space to become continuous with the inferior fascia of the diaphragm in bilateral kidneys.The anterior layers of the right renal fascia fused with the peritoneum of gallbladder neck or inferior margin of liver upwardly,and the posterior layers of the renal fascia fused with the subdiaphragmatic peritoneum.The anterior layers of left renal fascia fused with the fascia of pancreas,the peritoneum of inferior margin of spleen or inferior phrenic fascia upwardly,and superior margin of the posterior layers fused with the subdiaphragmatic fascia.The superior extent of the right perirenal space was open to the bare area of the liver.The left perirenal space was open towards the subdiaphragmatic space.Conclusion The locations of the superior attachment of the renal fascia are diversified,and MSCT and MPR images can clearly visualize renal fascia and its superior attachment and the superior communication of perirenal space.
