上尿路结石梗阻导致的尿源性脓毒血症与中医体质的相关性

摘　　要：目的探索影响上尿路结石梗阻导致的尿源性脓毒血症发病的相关体质因素,为尿源性脓毒血症的防治提供参考依据。方法收集2019年7月至2020年12月本院收治的64例上尿路结石梗阻导致的尿源性脓毒血症患者(病例组)及64例上尿路结石梗阻合并尿路感染患者(对照组)的中医体质调查问卷。采用1∶1配对的病例对照研究方法,采用Empowerstats 3.0进行条件单因素及多因素logistic回归分析。结果病例组各体质分布按照出现频数由高至低排列的前4位分别为:阳虚质、痰湿质、湿热质、阴虚质;通过单因素logistic回归分析结果显示,两组的阳虚质、痰湿质、气虚质患者比较,差异均有统计学意义(均P<0.05);多因素logistic回归分析结果显示,阳虚质是影响上尿路结石梗阻并发尿源性脓毒血症的危险因素(P<0.05)。结论上尿路结石梗阻导致的尿源性脓毒血症的发生与中医体质类型有相关性,提示需要综合防治上尿路结石梗阻并发尿源性脓毒血症。Objective To explore the relevant traditional Chinese medicine(TCM)constitution types affecting the development of urinary sepsis due to upper urinary tract stone obstruction and to provide references for the prevention and treatment of urinary sepsis.Methods The TCM constitution questionnaire was collected from 64 patients with urinary sepsis due to upper urinary tract stone obstruction(case group)and 64 patients with upper urinary tract stone obstruction combined with urinary tract infection(control group)admitted to our hospital from July 2019 to December 2020.A 1∶1 case-control analysis was used,and conditional univariate and multivariate logistic regression analyses were performed using Empowerstats 3.0.Results In the case group,the top four constitutions were Yang deficiency constitution,phlegm-dampness constitution,damp-heat constitution,and Yin deficiency constitution.Univariate logistic regression analysis showed that there were statistically significant differences in Yang deficiency,phlegm dampness and qi deficiency between the two groups(all P<0.05).Multivariate logistic regression analysis showed that Yang deficiency was a risk factor for upper urinary tract stone obstruction complicated with urosepsis(P<0.05).Conclusions The occurrence of urinary sepsis due to upper urinary tract stone obstruction is correlated with TCM constitution types,suggesting the need for comprehensive prevention and treatment of urinary sepsis complicated by upper urinary tract stone obstruction.
