保护性机械通气治疗新生儿脓毒血症合并急性呼吸窘迫综合征的临床效果

摘　　要：目的分析保护性机械通气在新生儿脓毒血症合并急性呼吸窘迫综合征中的应用价值。方法选取2020年1月至2022年1月邯郸市中心医院新生儿科收治的114例新生儿脓毒血症合并急性呼吸窘迫综合征患儿,根据患儿机械通气模式分组,其中观察组74例,对照组40例。观察组采用保护性机械通气模式,对照组则采取有创机械通气模式。比较两组并发症发生率差异,并比较两组治疗前后血气指标(氧分压、二氧化碳分压)与炎症指标(C反应蛋白、降钙素原)的变化。结果观察组并发症总发生率低于对照组(P<0.05);观察组治疗后氧分压、二氧化碳分压、C反应蛋白、降钙素原变化幅度高于对照组(P<0.05)。结论保护性机械通气的应用有助于改善患儿的血气指标,降低炎症反应,且安全性高,可推广应用。Objective To analyze the application value of protective mechanical ventilation in the treatment of neonatal sepsis complicated with acute respiratory distress syndrome.Methods A total of 114 cases of neonatal sepsis complicated with acute respiratory distress syndrome were selected from the Department of Neonatology of Handan Central Hospital from January 2020 to January 2022,and grouped according to the mechanical ventilation mode,including 74 cases in the observation group and 40 cases in the control group.The observation group was given protective mechanical ventilation mode,while the control group was given invasive mechanical ventilation mode.The incidence of complications was compared between the two groups,and the changes of blood gas indexes(partial pressure of oxygen,partial pressure of carbon dioxide)and inflammatory indexes(C-reactive protein,procalcitonin)before and after treatment were compared between the two groups.Results The total incidence of complications in the observation group was lower than that in the control group(P<0.05).The changes of partial pressure of oxygen,partial pressure of carbon dioxide,C-reactive protein and procalcitonin in the observation group after treatment were higher than those in the control group(P<0.05).Conclusion The application of protective mechanical ventilation is helpful to improve the blood gas index of children,reduce inflammation reaction,and has high safety,which can be popularized.
