摘 要:目的 探讨血清ET-1、NO水平联合肠道菌群对早期妊娠期高血压的诊断价值。方法 选取2019年8月至2020年12月河北北方学院附属第二医院收治的134例妊娠期高血压患者为研究组,该院同期收治的132例正常孕妇为对照组,比较两组血清ET-1、NO水平及肠道菌群(包括大肠杆菌、肠球菌、双歧杆菌及乳酸杆菌)菌落数。利用Logistic回归分析妊娠高血压的相关危险因素,ROC曲线评估血清ET-1、NO水平联合肠道菌群菌落数对早期妊娠高血压的可能诊断价值。结果 与对照组相比,研究组血清ET-1水平更高(P <0.05),NO水平更低(P <0.05);大肠杆菌及肠球菌菌落数明显更高(P <0.05),乳酸杆菌及双歧杆菌菌落数更低(P <0.05)。二元Logistic回归分析表明,血清ET-1、NO水平及大肠杆菌、乳酸杆菌菌落数为妊娠期高血压独立危险因素;联合检测血清ET-1、NO及大肠杆菌和乳酸杆菌的AUC为0.997。结论 单独检测孕妇血清ET-1、NO水平及肠道菌群菌落数可作为早期妊娠高血压诊断依据之一,联合检测血清ET-1、NO水平及肠道菌群菌落数可能增加诊断结果的确定性,值得临床推广应用。Objective To investigate the diagnostic value of serum ET-1,NO levels combined with intestinal fl ora in early pregnancy hypertension.Methods 134 patients with gestational hypertension who were admitted to the Second Affi liated Hospital of Hebei Northern University from August 2019 to December 2020 were selected as the research group,and 132 normal pregnant women during the same period were selected as the control group.The serum ET-1,NO level and colony count of intestinal fl ora(including Escherichia coli,Enterococcus,Bifi dobacterium and Lactobacillus) of the two groups was compared.Logistic regression was used to analyze the related risk factors of gestational hypertension,and ROC curve was used to evaluate the possible diagnostic value of serum ET-1 and NO levels combined with intestinal fl ora counts in early pregnancy hypertension.Results Compared with the control group,the serum ET-1 level in the study group was higher(P < 0.05),and the NO level was lower(P < 0.05).The colony counts of Escherichia coli and Enterococcus were signifi cantly higher(P < 0.05),and the colony counts of Lactobacillus and Bifi dobacterium were lower(P <0.05).Binary Logistic regression analysis showed that serum ET-1 and NO levels and the number of Escherichia coli and Lactobacillus colonies were independent risk factors for gestational hypertension;the combined detection of serum ET-1,NO and Escherichia coli and Lactobacillus AUC was 0.997.Conclusions The detection of serum ET-1 and NO levels and the number of intestinal fl ora colonies in pregnant women can be used as one indicator for the diagnosis of hypertension in early pregnancy.The combined detection of serum ET-1,NO levels and the number of intestinal fl ora colonies may increase the diagnostic effi ciency.The certainty is worthy of clinical application.
