摘  要:目的:探讨围手术期食管癌合并2型糖尿病患者采用强化营养支持护理联合肠内营养乳剂的价值。方法:选取福建医科大学附属第二医院胃肠外科2021年1月-2022年3月收治的121例接受根治性手术的食管癌合并2型糖尿病患者,随机分为对照组(60例)和观察组(61例)。对照组接受食管癌术后常规营养支持治疗,观察组给予强化营养支持护理联合肠内营养乳剂。比较两组血糖控制水平、营养指标、手术并发症和术后康复时间的差别。结果:术后7 d,观察组平均随机血糖为(7.63±0.82)mmol/L,低于对照组的(8.32±0.86)mmol/L,差异有统计学意义(t=14.043,P<0.05)。干预前,两组血浆白蛋白(ALB)和前白蛋白(PA)水平比较,差异无统计学意义(P>0.05),干预后,观察组ALB和PA水平均高于对照组,差异有统计学意义(P<0.05)。观察组术后并发症总发生率均低于对照组,差异有统计学意义(P<0.05)。观察组术后平均康复时间为(16.07±1.87)d,短于对照组的(17.80±1.67)d,差异有统计学意义(t=5.364,P<0.001)。结论:与常规营养支持治疗相比,强化营养支持护理联合肠内营养乳剂可以有效稳定食管癌合并2型糖尿病患者的血糖,改善营养指标,降低术后并发症发生率,缩短住院时间,值得推广应用。Objective:To investigate the value of intensive nutritional support nursing combined with enteral nutrition emulsion in perioperative esophageal cancer patients with type 2 diabetes.Method:A total of 121 patients with esophageal cancer and type 2 diabetes who underwent radical surgery in the Department of Gastroenterology of the Second Affiliated Hospital of Fujian Medical University from January 2021 to March 2022 were selected and randomly divided into control group(60 cases)and observation group(61 cases).The control group received routine nutritional support after esophageal cancer surgery,and the observation group was given intensive nutritional support nursing combined with enteral nutrition emulsion.The differences of blood glucose,nutritional indicators,surgical complications and postoperative recovery time between the two groups were compared.Result:At 7 d after operation,the average random blood glucose in the observation group was(7.63±0.82)mmol/L,which was lower than(8.32±0.86)mmol/L in the control group,and the difference was statistically significant(t=14.043,P<0.05).Before the intervention,there were no significant differences between the two groups in plasma albumin(ALB)and prealbumin(PA)levels(P>0.05).After the intervention,the levels of ALB and PA in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The average postoperative recovery time in the observation group was(16.07±1.87)d,which was shorter than(17.80±1.67)d in the control group,and the difference was statistically significant(t=5.364,P<0.001).Conclusion:Compared with conventional nutritional support treatment,intensive nutritional support nursing combined with enteral nutrition emulsion can effectively stabilize blood sugar in patients with esophageal cancer complicated with type 2 diabetes,im
