摘 要:目的探讨边疆少数民族地区世居彝族人群染色体9p21上rs1333049单核苷酸多态性(SNP)与冠心病的联系。方法采用病例对照研究,随机选取红河哈尼族彝族自治州各县市2018年4月至2020年12月在昆明医科大学第五附属医院住院的120例年龄>18岁,3代以内均居住在边疆本地的世居彝族人群作为研究对象,平均年龄(56.83+10.29)岁,根据临床资料、冠状动脉造影及相关辅助检查分为冠心病组和对照组,每组各60例。收集个人资料及相关辅助检查指标,采用实时荧光定量聚合酶链反应检测rs1333049 SNP并分析基因型。结果冠心病组和对照组人群平均年龄分别为(58.62±9.05)岁、(55.05±11.19)岁;男性占比分别为58.3%、50.0%;冠心病组和对照组高血压、糖尿病、吸烟、饮酒、空腹血糖(FBG)比较,差异均有统计学意义(P<0.05);不同性别、不同年龄(≤45岁、>45~65岁、>65岁)人群rs1333049基因型(CC、CG、GG)频率比较,差异均无统计学意义(P>0.05);冠心病组rs1333049的CG基因型频率(58.3%)高于对照组(36.7%),差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,校正了高血压、糖尿病、吸烟、饮酒后,年龄(OR=1.054,95%CI:1.009~1.101)、FBG(OR=1.741,95%CI:1.156~2.623)、rs1333049的CG基因型(OR=4.160,95%CI:1.399~12.370)为彝族冠心病的影响因素(P<0.05);年龄>61.5岁,FBG>6.06 mmol/L预测彝族人群患冠心病的受试者工作特征曲线下面积分别为0.615、0.644,其灵敏度分别为45.0%、41.7%,特异度分别为87.3%、89.9%。结论rs1333049 CG基因型为彝族冠心病的易感基因型,结合年龄、入院FBG水平及rs1333049基因检测可评估彝族人群患冠心病的风险。Objective To explore the relationship between the single nucleotide polymorphism(SNP)of rs1333049 on chromosome 9P21 of Yi people living in frontier minority areas and coronary heart disease.Methods Using the case-control method,120 indigenous Yi people aged over 18 years who were hospitalized in the Fifth Affiliated Hospital of Kunming Medical University from April 2018 to December 2020 in all counties and cities of Honghe Hani and Yi Autonomous Prefecture were randomly selected as the research objects,all of them were native ethnic minority within three generations and living in Honghe Hani and Yi Autonomous Prefecture.The average age of them was(56.83±10.29)years old,according to clinical data,coronary angiography and related auxiliary examinations,they were divided into coronary heart disease group and control group,with 60 cases in each group.Personal data and related auxiliary inspection indicators were collected,real-time fluorescence quantitative TaqMan technology was used to detect rs1333049 SNP and analyze genotypes.Results The average age of the two groups of people were(58.62±9.05)years and(55.05±11.19)years,of which male accounted for 58.3%and 50.0%.Hypertension,diabetes,smoking,drinking,fasting blood glucose(FBG)of two groups had statistically significant differences(P<0.05),there was no significant difference in frequency of rs1333049 genotype(CC,CG,GG)in different genders and different age groups(≤45 years old,>45-65 years old,>65 years old)(P>0.05).The frequency of CG genotype of rs1333049(58.3%)in coronary heart disease group was higher than that of the control group(36.7%),and the difference was statistically significant(P<0.05).Multivariate Logistic regression analysis showed that,after correcting for hypertension,diabetes,smoking/drinking history,age(OR=1.054,95%CI:1.009-1.101),FBG(OR=1.741,95%CI:1.156-2.623),rs1333049 CG genotype(OR=4.160,95%CI:1.399-12.370)were independent risk factors for coronary heart disease in Yi people(all P<0.05).The area under the curve(AUC)of ROC curve showed
