摘  要:目的探讨血清硫酸基转移酶2A1(SULT2A1)、Betatrophin、蛋白聚糖-1(syndecan-1)与妊娠期肝内胆汁淤积症(ICP)患者肝功能的关系及对围生儿结局的影响。方法选取2019年4月至2021年10月西安经开妇幼医院收治的105例ICP患者作为研究组;另选取同期100例体检健康孕妇作为对照组。比较研究组和对照组血清SULT2A1、Betatrophin、syndecan-1及肝功能指标水平。采用Pearson相关分析血清SULT2A1、Betatrophin、syndecan-1水平与各项肝功能指标的相关性。将研究组患者按照围生儿结局分为不良结局组(43例)和良好结局组(62例)。比较不良结局组和良好结局组血清SULT2A1、Betatrophin、syndecan-1水平。采用单因素、多因素Logistic回归分析ICP患者不良围生儿结局的影响因素。结果研究组血清SULT2A1水平低于对照组,而Betatrophin、syndecan-1水平均高于对照组,差异均有统计学意义(P<0.05)。研究组天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)及碱性磷酸酶(ALP)水平均高于对照组,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,血清SULT2A1水平与AST、ALT、ALP水平均呈负相关(P<0.05),而Betatrophin、syndecan-1水平与AST、ALT、ALP水平均呈正相关(P<0.05)。不良结局组血清SULT2A1水平低于良好结局组,而Betatrophin、syndecan-1水平均高于良好结局组,差异均有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,血清SULT2A1水平降低(≤23μmol/L)、Betatrophin水平升高(≥650 pg/mL)及syndecan-1水平升高(≥53 ng/mL)为ICP患者不良围生儿结局的危险因素(P<0.05)。结论血清SULT2A1、Betatrophin、syndecan-1与ICP患者肝功能及围生儿结局均密切相关,即随着血清SULT2A1水平降低及Betatrophin、syndecan-1水平升高,可导致ICP患者肝功能受损加重,引发不良围生儿结局。Objective To investigate the relationship between serum sulfated transferase 2A1(SULT2A1),Betatrophin,syndecan-1 and liver function in patients with intrahepatic cholestasis of pregnancy(ICP)and the effects on perinatal outcomes.Methods A total of 105 patients with ICP who were admitted to Xi′an Jingkai District Women′s and Children′s Hospital from April 2019 to October 2021 were selected as the study group.Another 100 healthy pregnant women during the same period were selected as the control group.Serum SULT2A1,Betatrophin,syndecan-1 levels and liver function indexes were compared between the two groups.Pearson correlation analysis was used to analyze the correlation between serum SULT2A1,Betatrophin,syndecan-1 levels and various liver function indexes.According to perinatal outcomes,the patients in the study group were divided into adverse outcome group(n=43)and good outcome group(n=62).Serum SULT2A1,Betatrophin and syndecan-1 levels were compared between the two groups.Univariate and multivariate Logistic regression were used to analyzed the influencing pactors adverse perinatal outcomes in patients with ICP.Results The serum SULT2A1 level in the study group was significantly lower than that in the control group(P<0.05).The Betatrophin and syndecan-1 levels were significantly higher than those in the control group(P<0.05).The aspartate aminotransferase(AST),alanine aminotransferase(ALT)and alkaline phosphatase(ALP)levels in the study group were higher than those in the control group(P<0.05).Pearson correlation analysis showed that serum SULT2A1 level was negatively correlated with AST,ALT and ALP levels,while Betatrophin,syndecan-1 levels were positively correlated with AST,ALT and ALP levels(P<0.05).The serum SULT2A1 level in the adverse outcome group was lower than that in the good outcome group,while the Betatrophin and syndecan-1 levels were higher than those in the good outcome group(P<0.05).Multivariate Logistic regression analysis showed that decreased serum SULT2A1 level(≤23μmol/L),increased B
