Ilizarov外固定架骨延长联合皮瓣移植治疗长段胫骨骨缺损的研究

摘　　要：目的应用Ilizarov外固定架骨延长技术结合局部转移或游离皮瓣对创伤患者的胫腓骨骨缺损和软组织缺损进行治疗研究,探讨该技术的临床疗效及安全性。方法选取2020年1月—2021年12月齐齐哈尔市第一医院骨科收治的创伤性大段胫骨骨缺损伴软组织缺损患者60例为研究对象,随机分为两组。治疗组30例采用Ilizarov外固定架结合局部转移皮瓣或游离皮瓣移植治疗,对照组30例采用传统大段髂骨植骨或腓骨植骨治疗。比较术后两组临床疗效和踝关节功能。结果治疗组治疗优良率为86.67%,高于对照组的63.33%,差异有统计学意义(χ^(2)=4.356,P<0.05);治疗组患者的踝关节功能、活动、步态、疼痛评分均高于对照组,差异有统计学意义(P<0.05)。结论Ilizarov外固定架骨延长技术可提高长段骨缺损合并软组织缺损患者的临床疗效,改善其踝关节功能。Objective To apply the Ilizarov external fixator bone lengthening technique combined with flap graft to treat tibiofibular bone defects and soft tissue defects in trauma patients and to investigate the clinical efficacy and safety of this technique.Methods From January 2020 to December 2021,60 patients with traumatic large tibial bone defects and soft tissue defects admitted to the Department of Orthopedics of the Qiqihar First Hospital were selected as the research objects.The patients were randomly divided into two groups.30 cases in the treatment group treated with Ilizarov external fixation frame combined with local transfer flap or free flap graft and 30 cases in the control group were treated with traditional large segment iliac bone graft or fibula bone graft.Comparison of clinical efficacy and ankle function between the two groups.Results The excellent and good rate of the treatment group was 86.67%,which was higher than 63.33%of the control group,and the difference was statistically significant(χ^(2)=4.356,P<0.05).The ankle function,activity,gait and pain scores of patients in the treatment group were higher than those of patients in the control group,and the difference was statistically significant(P<0.05).Conclusion Ilizarov external fixator bone lengthening technology can improve the clinical efficacy of patients with long bone defects and soft tissue defects,and improve the ankle joint function.
