经阴道超声检查诊断早期异位妊娠的临床价值探讨

摘　　要：目的探讨对早期异位妊娠采用经阴道超声检查诊断的价值。方法选择2020年1月—2021年12月丹阳市人民医院接收的120例疑似异位妊娠者,均进行经腹超声与经阴道超声检查。比较两种检查方式对异位妊娠典型超声征象的检出情况,并以临床检查结果为金标准,评估经腹部与经阴道超声的诊断效果。结果经阴道超声对心管搏动、假孕囊、盆腔积液、胚芽以及包块的检出率分别为10.00%、25.83%、80.83%、22.50%、76.67%,均高于经腹超声检查的3.33%、6.67%、59.17%、5.00%、56.67%,差异有统计学意义(χ^(2)=6.125、23.000、26.000、21.000、24.000,P<0.05)。经腹超声诊断早期异位妊娠的准确率、灵敏度、特异度分别为77.50%、76.47%、83.33%,其诊断结果与临床检查结果一致性一般(Kappa=0.40)。经阴道超声诊断早期异位妊娠的准确率、灵敏度、特异度分别为95.00%、95.10%、94.44%,其诊断结果与临床检查结果比较一致性较好(Kappa=0.82)。结论对早期异位妊娠采用经阴道超声检查能够有效识别患者的超声征象,诊断结果与临床检查的一致性好,诊断价值高。Objective To explore the value of using transvaginal ultrasonography for the diagnosis of early ectopic pregnancy.Methods From January 2020 to December 2021,120 suspected ectopic pregnancies received by Danyang People's Hospital were examined by transabdominal ultrasound and transvaginal ultrasound.To compare the detection of typical ultrasonic signs of ectopic pregnancy by the two methods,take clinical examination results as the gold standard,and to evaluate the diagnostic effect of transabdominal and transvaginal ultrasound.Results The detection rates of transvaginal ultrasound for cardiac pulsation,pseudogestational sac,pelvic effusion,embryo and mass were 10.00%,25.83%,80.83%,22.50%and 76.67%,respectively,which were higher than those of transabdominal ultrasound for 3.33%,6.67%,59.17%,5.00%and 56.67%,and the difference was statistically significant(χ^(2)=6.125,23.000,26.000,21.000,24.000,P<0.05).The accuracy,sensitivity and specificity of transabdominal ultrasound in the diagnosis of early ectopic pregnancy were 77.50%,76.47%and 83.33%,respectively.The diagnostic results were generally consistent with the clinical examination results(Kappa=0.40).The accuracy,sensitivity and specificity of transvaginal ultrasound in the diagnosis of early ectopic pregnancy were 95.00%,95.10%and 94.44%,respectively,the diagnostic results were in good agreement with the clinical examination results(Kappa=0.82).Conclusion Transvaginal ultrasound can effectively identify the ultrasonic signs of patients with early ectopic pregnancy,and the diagnostic results are consistent with clinical examination,with high diagnostic value.
