七氟烷联合不同剂量芬太尼舒适化麻醉技术对小儿日间手术的安全性影响分析

摘　　要：目的评估日间手术患儿实施七氟烷联合不同剂量芬太尼舒适化麻醉技术的安全性。方法选择2020年6月—2021年6月四川省绵阳市中心医院的78例实施日间手术治疗的患儿为研究对象,以随机数表法为分为七氟烷麻醉组(n=26)、七氟烷与1μg/kg芬太尼联合麻醉组(n=26)、七氟烷与1.5μg/kg芬太尼联合麻醉组(n=26),对比组间体征参数、临床指标及不良反应情况。结果与七氟烷组对比,七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组的患儿平均动脉压、心率较低、血氧饱和度较高,差异有统计学意义(P<0.05)。七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组的血氧饱和度和呼气末二氧化碳对比,差异无统计学意义(P>0.05)。与七氟烷组对比,七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组的患儿七氟烷用量较少、苏醒时长较短,差异有统计学意义(P<0.05)。七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组的七氟烷用量、苏醒时长对比,差异无统计学意义(P>0.05)。七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组的总不良反应发生率(19.23%、15.38%)较低于七氟烷组(57.69%),差异有统计学意义(χ^(2)=8.125、10.035,P<0.05)。七氟烷与1μg/kg芬太尼联合麻醉组、七氟烷与1.5μg/kg芬太尼联合麻醉组不良反应发生率对比,差异无统计学意义(χ^(2)=0.000,P>0.05)。结论日间手术患儿实施七氟烷联合不同剂量芬太尼舒适化麻醉技术效果显著,其麻醉后舒适度及安全性水平相对较高。Objective To evaluate the safety of sevoflurane combined with different doses of fentanyl comfort anesthesia technique in children undergoing day surgery.Methods A total of 78 children undergoing day surgery in Mianyang Central Hospital of Sichuan Province from June 2020 to June 2021 were selected as the research objects.They were divided into sevoflurane anesthesia group(n=26),sevoflurane and 1μg/kg fentanyl combined anesthesia group(n=26),sevoflurane and 1.5μg/kg fentanyl combined anesthesia group(n=26)by random number table.The physical parameters,clinical indicators and adverse reactions were compared between the groups.Results Sevoflurane and 1μg/kg fentanyl combined anesthesia group,sevoflurane and 1.5μg/kg fentanyl combined anesthesia group of children compared with sevoflurane group of children with mean arterial pressure,heart rate were low,blood oxygen saturation were higher,the difference was statistically significant(P<0.05).There was no statistically significant difference in blood oxygen saturation and end expiratory carbon dioxide between,sevoflurane and 1μg/kg fentanyl combined anesthesia group,and sevoflurane and 1.5μg/kg fentanyl combined anesthesia group(P>0.05).Compared with the sevoflurane group,the sevoflurane combined with 1μg/kg fentanyl anesthesia group and the sevoflurane combined with 1.5μg/kg fentanyl anesthesia group had less sevoflurane dosage and shorter recovery time,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the amount of sevoflurane and the recovery time between the sevoflurane and 1μg/kg fentanyl combined anesthesia group and the sevoflurane and 1.5μg/kg fentanyl combined anesthesia group(P>0.05).The incidence of adverse reation of sevoflurane combined with 1μg/kg fentanyl anesthesia group and sevoflurane combined with 1.5μg/kg fentanyl anesthesia group(19.23%,15.38%)were lower than sevoflurane group(57.69%),and the difference was statistically significant(χ^(2)=8.125,10.035,P<0.05).There was no stat
