摘　　要：目的本文研究老年人髋关节置换术中罗哌卡因腰硬联合麻醉法的效果和安全。方法选取2019年3月-2021年3月本院收治的100例髋关节置换术患者,随机进行分组,即为对照组和研究组,每组各50例。对照组采用常规硬膜外麻醉,研究组采用罗哌卡因腰硬联合麻醉。收集并分析患者的麻醉优良率、心率、平均动脉压、血氧饱和度、麻醉时间情况、术后满意度、麻醉并发症等数据。结果研究组麻醉优良率高于对照组(P<0.05)。研究组患者手术期间心率、平均动脉压、血氧饱和度等指标平稳程度优于对照组(P<0.05)。研究组患者麻醉起效时间、感觉阻滞起效时间、运动阻滞起效时间低于对照组,镇痛持续时间高于对照组(P<0.05)。研究组满意度高于对照组(P<0.05)。研究组麻醉并发症数量低于对照组(P<0.05)。结论在为老年人髋关节置换术患者进行麻醉时,通过罗哌卡因腰硬联合麻醉法能够有效提高麻醉效果,患者麻醉期间各项生命体征指标更加平稳,麻醉起效时间更快,麻醉安全性更高,建议在实际的手术麻醉工作中予以推广。Objective To study the efficacy and safety of ropivacaine combined lumbar and epidural anesthesia in elderly hip arthroplasty.Methods The research period was from March 2019 to March 2021.Subjects:100 hip replacement patients were collected from our hospital and randomly divided into groups.The number of patients in each group was 50.The control group received conventional epidural anesthesia.Combined lumbar epidural anesthesia with ropivacaine was used in the study group.Finally,the excellent rate of anesthesia,heart rate,mean arterial pressure,oxygen saturation,anesthesia time,postoperative satisfaction,anesthesia complications and other data were collected and analyzed.Results The excellent and good rate of anesthesia in the study group was higher than that in the control group,P<0.05.The stability of heart rate,mean arterial pressure and blood oxygen saturation during operation in the study group was better than that in the control group.The effective time of anesthesia,sensory block and motor block in the study group was lower than that in the control group,and the duration of analgesia was higher than that in the control group,P<0.05.The satisfaction of study group was higher than that of control group(P<0.05).The number of anesthesia complications in the study group was lower than that in the control group,P<0.05.Conclusion Ropivacaine combined lumbar and epidural anesthesia can effectively improve the anesthetic effect in elderly patients with hip replacement.The vital signs of patients during anesthesia are more stable,the effective time of anesthesia is faster,and the safety of anesthesia is higher.It is suggested to be popularized in practical surgical anesthesia.
