机械性血栓清除术联合髂静脉支架植入术对左下肢静脉血栓的疗效

摘　　要：目的探讨机械性血栓清除术联合一期髂静脉支架植入术一站式治疗髂静脉受压综合征合并急性左下肢深静脉血栓形成的效果。方法回顾性分析本院收治的36例髂静脉受压综合征合并急性左下肢深静脉血栓形成的患者临床资料。其中11例采用机械性血栓清除术(PMT),联合一期髂静脉支架植入术治疗(A组);25例采用导管接触溶栓(CDT)或大腔导管血栓抽吸术(MAT),联合分期髂静脉支架植入术治疗(B组)。比较两组间血栓清除率、肢体消肿率与消肿时间、并发症发生率及支架通畅率、PTS发生率。结果两组患者良好血栓清除率、显效消肿率、严重并发症比较无明显差异,消肿时间两组间有统计学差异(t=-4.597,P=0.01)。支架通畅率、PTS发生率、Villalta评分两组比较,无显著差异(P>0.05)。结论机械性血栓清除术联合一期髂静脉支架植入术一站式治疗髂静脉受压综合征合并急性左下肢深静脉血栓形成安全有效。Objective To investigate the effect of mechanical thrombectomy combined with onestage iliac vein stenting in the treatment of iliac vein compression syndrome complicated with acute left lower extremity deep vein thrombosis.Methods The clinical data of 36 patients with iliac vein compression syndrome combined with acute left lower extremity deep vein thrombosis in our hospital were retrospectively analyzed.Among them,11 cases were treated with mechanical thrombectomy(PMT)combined with one-stage iliac vein stenting(group A);25 cases were treated with cathetercontact thrombolysis(CDT)or large-lumen catheter thrombus aspiration(MAT).Combined staged iliac vein stenting treatment(group B).The thrombus clearance rate,limb swelling rate and swelling time,complication rate,stent patency rate,and PTS incidence were compared between the two groups.Results There was no significant difference in thrombus clearance rate,limb swelling rate and complication rate between the two groups.There was a statistically significant difference in the detumescence time between the two groups(t=-4.597,P=0.01).There was no significant difference in the stent patency rate and the incidence of PTS between the two groups.Conclusion Mechanical thrombectomy combined with one-stage iliac vein stenting is safe and effective in the treatment of iliac vein compression syndrome complicated with acute left lower extremity deep vein thrombosis.
