清创术联合负压封闭引流技术治疗糖尿病足创面的临床观察

摘　　要：目的观察清创术联合负压封闭引流技术治疗糖尿病足创面的临床疗效。方法本院2019年6月-2020年6月收治的62例糖尿病足创面患者为本次研究对象,按照是否开展清创术联合负压封闭引流技术将患者分为对照组(31例:未采取上述联合治疗方案,单纯开展清创术)与实验组(31例:开展上述联合治疗方案),比较两组患者治疗效果。结果实验组患者术后细菌定量以及术后疼痛感均低于同一时间对照组,数据差异明显(P<0.05)。实验组患者治疗2周后巨噬细胞为(4.03±0.11)个、白细胞为(1.65±0.11)个、淋巴细胞为(3.21±0.21)个、中性粒细胞百分比(60.21±4.45)%,相关细胞因子均优于对照组,实验组治疗2周后C反应蛋白为(8.38±1.42)mg/L、白细胞介素-1为(5.23±0.15)ng/L、白细胞介素-6为(38.54±3.11)pg/L、肿瘤坏死因子-α为(45.29±4.45)pg/L,相关炎症因子均低于对照组,数据差异明显(P<0.05)。实验组患者术后2周治疗总有效率(96.77%)以及患者对治疗效果满意度(93.55%)均显著高于对照组,数据差异明显(P<0.05)。结论糖尿病足创面患者清创术联合负压封闭引流技术治疗效果显著优于单纯清创术治疗效果。Objective To observe clinical curative effect of debridement combined with vacuum sealing drainage in treatment of diabetes foot wounds.Methods The paper chose 62 patients with diabetes foot wounds admitted to our hospital from June 2019 to June 2020 as study subjects,and divided them into control group(31 cases:with no combined treatment scheme,only debridement)and experimental group(31 cases:with combined treatment scheme)based on whether carrying out debridement combined with negative pressure sealing drainage technology.Treatment effect between two groups was compared.Results Quantity of bacteria and postoperative pain in experimental group were lower than control group at the same time,with significant difference(P<0.05).After 2 weeks of treatment,number of macrophages,leukocytes,lymphocytes and neutrophils in experimental group were(4.03±0.11),(1.65±0.11),(3.21±0.21),and(60.21±4.45)%respectively.Related cytokines in experimental group were better than control group.After 2 weeks of treatment,C-reactive protein,interleukin-1,interleukin-6 and tumor necrosis factor-αin experimental group were(8.38±1.42)mg/L,(5.23±0.15)ng/L,(38.54±3.11)pg/L,and(38.54±3.11)pg/L,(45.29±4.45)pg/L respectively,related inflammatory factors were lower than control group,with significant difference(P<0.05).Total effective rate(96.77%)and satisfaction degree(93.55%)of experimental group were significantly higher than control group 2 weeks after operation,with significant difference(P<0.05).Conclusion Debridement combined with vacuum sealing drainage can achieve significantly better effect for patients with diabetes foot wounds than pure debridement only.
