应对方式在突发公共卫生事件下医务人员心理一致感与创伤后应激中的双重中介作用

摘　　要：目的探索心理一致感对医务人员突发公共卫生事件创伤后应激反应的影响应对方式的双重中介作用。方法回顾性分析的69例医务人员调查内容均在2020年1月-2021年5月期间收集,均采用了心理一致感量表(SOC-13)、事件影响量表-修订表(IES-R)、医学应对方式量表评估,再分别采用Pearson法、Bootstrap法分析三者相关性。结果经Pearson法分析,心理一致感与屈服、回避、高警觉、回避反应、闯入性体验呈负相关性,与面对呈正相关性。同时运用Bootstrap抽样检验法进行中介效应分析:高警觉→面对→心理一致感(95%CI:0.004~0.091)此条路径存在中介效应。链式中介效应路径进行分析:闯入性体验→屈服→回避→心理一致感、闯入性体验→回避→心理一致感、闯入性体验→屈服→心理一致感、回避反应→屈服→回避→心理一致感、回避反应→屈服→心理一致感、高警觉→面对→心理一致感、高警觉→回避→心理一致感,以上路径存在中介效应。结论医务人员心理一致感与突发公共卫生事件创伤后应激反应存在负相关性。而应对方式在医务人员创伤后应激反应和心理一致感之间起到中介作用。Objective To explore the dual mediating effect of psychological coherence on the post-traumatic stress response of medical staff after public health emergencies.Methods The survey contents of 69 medical staff in a retrospective analysis were collected from January 2020 to May 2021.The medical coping style scale assessment was evaluated by the Psychological Consistency Scale(SOC-13),the Event Impact Scale Revised Scale(IES-R),and the Medical Coping Style Scale,and then the Pearson method and the Bootstrap method was used to analyze the correlation of the three.Results According to the analysis of Pearson method,psychological consistency is negatively correlated with submission,avoidance,high alertness,avoidance response,and intrusive experience,and positively correlated with confrontation.At the same time,Bootstrap sampling inspection method was used to analyze the mediation effect:high alert→face→psychological agreement(95%CI:0.004~0.091).This path has a mediation effect.Analysis of the chain-like mediation effect path:intrusive experience→yielding psychological coherence,avoidance reaction→yield→psychological coherence,high alertness→face→psychological coherence,high alertness→avoidance→psychological coherence,the above path has a mediating effect.Conclusion There is a negative correlation between the psychological consistency of medical staff and the post-traumatic stress response of public health emergencies.The coping style plays a mediating role between the post-traumatic stress response and psychological coherence of medical staff.
