摘　　要：目的分析白介素6(IL-6)和脂蛋白相关磷脂酶A2(Lp-PLA2)与非瓣膜性心房颤动(NVAF)的关系。方法选取2020年1~12月在泰州市人民医院心内科就诊的NVAF患者150例作为实验组,同时期住院的窦性心律患者150例作为对照组。收集患者的临床资料,检测血清中IL-6和Lp-PLA2水平,采用多元Logistic回归分析及受试者工作特征(ROC)曲线分析其与NVAF的相关性。结果与对照组相比,实验组的红细胞分布宽度(RDW)、中性粒细胞与淋巴细胞比值(NLR)、低密度脂蛋白(LDL)、IL-6、Lp-PLA2、左房内径(LA)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)水平更高,差异有统计学意义(P<0.05)。Logistic回归分析显示,RDW、IL-6、Lp-PLA2和LA是NVAF发生的独立危险因素(P<0.05)。ROC曲线分析发现,IL-6、Lp-PLA2、RDW和LA的曲线下面积分别为0.705、0.716、0.701、0.820,对NVAF均有一定的预测价值。结论IL-6、Lp-PLA2、RDW和LA与NVAF发生有关,可辅助诊断NVAF,为NVAF的早期诊断和干预提供了新思路。Objective This article aims to analyze the relationship between interleukin-6(IL-6)and lipoprotein-associated phospholipase A2(Lp-PLA2)in non-valvular atrial fibrillation(NVAF).Methods 150 patients with NVAF in the Department of Cardiology of Taizhou People's Hospital from January to December 2020 were selected as the experimental group,and 150 patients with sinus rhythm hospitalized in the same period were selected as the control group.The clinical data were collected,for detection of serum IL-6 and Lp-PLA2 levels.Multiple Logistic regression analysis and receiver operating characteristic(ROC)curve were used to analyze its correlation with NVAF.Results Compared with the control group,red blood cell distribution width(RDW),neutrophil-to-lymphocyte ratio(NLR),low-density lipoprotein(LDL),IL-6,Lp-PLA2,left atrial diameter(LA),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF)were higher in the experimental group and the difference was statistically significant(P<0.05).Logistic regression analysis showed that RDW,IL-6,Lp-PLA2 and LA were independent risk factors for NVAF(P<0.05).The ROC curve discover that the areas under the curve of IL-6,Lp-PLA2,RDW and LA were 0.705,0.716,0.701,and 0.820,respectively,which had certain predictive value for NVAF.Conclusion IL-6,Lp-PLA2,RDW and LA are all risk factors for NVAF and have predictive value,providing new ideas for the early diagnosis and intervention of NVAF.
