《临证指南医案》补益类动物药应用规律分析

摘　　要：目的总结《临证指南医案》中补益类动物药的用药规律,解析叶天士治疗思想及用药特点。方法利用中医传承计算平台V3.0系统,收集《临证指南医案》中含有补益类动物药的处方,对处方所含药物的四气、五味、归经、功效进行分析,对处方间药物的关联规则、组合频次等进行分析。结果共纳入处方641首,补益类动物药四气以平(34.98%)、寒(31.88%)、温(28.88%)为主,五味以甘(51.66%)、咸(44.55%)为主,归经以肾经(36.54%)、肝经(33.27%)为主,功效以补虚类药物(47.56%)为主。组方以阿胶—地黄为主,其次有柴胡—鳖甲、鹿茸—当归等。结论叶氏在具体应用补益类动物药时以补虚类药物为主,同时配伍清热药、安神药等,以内科病中的脏腑虚证为主,同时注重妇科病的填补为用。Objective To systematically summarize the medication rules of tonic animal medicines in Case Records as a Guide to Clinical Practice,and analyze YE Tian-shi′s treatment ideas and medication characteristics.Methods The Traditional Chinese Medicine Inheritance Support System(V3.0)was used to collect the prescriptions containing tonic animal medicines in Case Records as a Guide to Clinical Practice,and the four properties,five flavors,meridian tropism,and efficacies of medicines in the prescriptions were analyzed.The association rules and combination frequency of medicines among prescriptions were analyzed.Results A total of 641 prescriptions were included.The four properties of tonic animal medicines were mainly flat(34.98%),cold(31.88%),and warm(28.88%).The five flavors were mainly sweet(51.66%)and salty(44.55%).The meridian tropisms were mainly renal meridian(36.54%)and liver meridian(33.27%),and the efficacy was mainly tonifying deficiency(47.56%).The prescriptions were mainly Asini Corii Colla-Rehmanniae Radix medicine pair,followed by Bupleuri Radix-Trionycis Carapax medicine pair and Cervi Cornu Pantotrichum-Angelicae Sinensis Radix.Conclusions In the specific application of tonic animal medicines,YE Tian-shi mainly used medicines with deficiency-tonifying efficacy while adding heat-clearing medicines and tranquilizers,mainly focusing on the zang-fu deficiency of internal diseases and treating gynaecopathia by tonification.
