颞三针联合熄风化痰通髓汤对中风恢复期患者肢体运动及平衡功能的影响

摘　　要：目的观察颞三针联合熄风化痰通髓汤治疗风痰瘀阻型中风恢复期患者对其肢体运动与平衡功能的影响,旨在为中风恢复期患者的治疗提供新思路。方法选取2017年2月—2020年11月期间马鞍山市中医院康复4科收治的128例中风恢复期患者,根据患者或家属选取的治疗方案分为观察组和对照组,每组各64例。对照组常规治疗+康复训练治疗,观察组常规治疗+康复训练+熄风化痰通髓汤+颞三针治疗。治疗3周后,观察比较两组患者治疗前后中医证候、肢体运动功能(Fugl-Meyer运动功能评分法评估)、平衡功能(采用Berg量表评估)、日常生活功能(Barthel生活指数评定量表)评分情况。结果治疗后两组患者中医证候积分均较治疗前降低,差异有统计学意义(P<0.05);且观察组中医证候积分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者Fugl-Meyer运动评分均较治疗前升高,差异有统计学意义(P<0.05);且观察组Fugl-Meyer评分明显高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者Berg量表评分均较治疗前升高,差异有统计学意义(P<0.05);且观察组Berg量表评分明显高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者Barthel生活指数评分均较治疗前升高,差异有统计学意义(P<0.05);且观察组Barthel生活指数评分明显高于对照组,差异有统计学意义(P<0.05)。结论在常规治疗基础上,颞三针联合熄风化痰通髓汤治疗可有效改善中风恢复期患者肢体运动、平衡功能、日常生活能。Objective To use Xifeng Huatan Tongsui Decoction(XHTD)and temporal three-needle therapy(TTNT)for the treatment of patients recovering from stroke with wind-phlegm and blood stasis,to explore the effect of the combination on the limb movement and balance function of patients,and thus to provide new ideas for the treatment of patients recovering from stroke.Methods A total of 128 patients recovering from stroke who were treated in Rehabilitation Department,Maanshan Hospital of TCM from February 2017 to November 2020 were included and classified into control group(64 cases,conventional treatment+rehabilitation training)and observation group(64 cases,conventional treatment+rehabilitation training+XHTD+TTNT)based on the therapy they chose.After three weeks of intervention,the traditional Chinese medicine(TCM)syndrome score,limb motor function[Fugl-Meyer motorscale(FM)score],balance function(Ber scale score),and daily living function(Barthelscale score)were compared between the two groups before and after treatment.Results After treatment,the TCM syndrome score decreased in both groups as compared with that before treatment(P<0.05)and the score was lower in the observation group than in the control group(P<0.05).FM score was higher than that before intervention in both groups(P<0.05),and higher in the observation group than in the control group(P<0.05).The Ber scale score increased after treatment in the two groups(P<0.05)and the score in the observation group was higher than that in the control group(P<0.05).Barthel scale score increased in both groups after treatment(P<0.05)and the score was higher in the observation group than in the control group(P<0.05).Conclusion On the basis of conventional treatment,the combination of XHTD and TTNT significantly improves the limb movement,balance function,and daily living ability of patients recovering from stroke,which can improve the prognosis of patients and is worthy of clinical promotion.
