摘 要:目的观察Ilizarov技术联合康复训练治疗中老年膝骨关节炎(KOA)的临床疗效。方法将37例中老年KOA患者予Ilizarov技术联合康复训练治疗。分别于术前及术后3、6、12个月比较患者美国膝关节外科学会膝关节功能评分(KSS)、膝关节活动度(ROM)、美国特种外科医院膝关节功能评分(HSS)、疼痛视觉模拟评分(VAS)及下肢负重位力学轴线指标髋-膝-踝角(HKA)和胫股角(FTA)。结果37例患者术后3、6、12个月疼痛、缺陷扣分评分均较术前下降(P<0.05),稳定性、活动度评分及总评分均较术前升高(P<0.05),且术后3、6、12个月两两比较差异均有统计学意义(P<0.05)。37例患者术后3、6、12个月ROM、HSS均较术前升高(P<0.05),疼痛VAS较术前下降(P<0.05),且术后3、6、12个月两两比较差异均有统计学意义(P<0.05)。37例患者术后3、6、12个月HKA较术前升高(P<0.05),FTA较术前降低(P<0.05),且术后3、6、12个月两两比较差异均有统计学意义(P<0.05)。结论Ilizarov技术联合康复训练治疗中老年KOA,能缓解患者疼痛症状,提高膝关节活动度,改善膝关节稳定性,优化下肢负重位力学轴线,促进膝关节功能恢复。Objective To observe the clinical effect of Ilizarov technology combined with rehabilitation training for middle-aged and elderly patients with knee osteoarthritis(KOA).Methods Totally 37 middle-aged and elderly patients with KOA were managed by Ilizarov technology combined with rehabilitation training.The aim was to compare American Knee Society Scores(KSS),knee range of motion(ROM),Hospital for Special Surgery(HSS)knee score,visual analogue scale(VAS),The mechanical axis of the lower limb(MAL)including hip-knee-ankle angle(HKA)and the femorotibial angle(FTA)during preoperative period and months 3,6,12.Results Among 37 patients,the postoperative pain and defection subtraction score were decreased(P<0.05);stability and activity score,and their total score were increased(P<0.05),the difference was statistically significant in above data between month 3 and month 6,month 3 and month 12,month 6 and month 12(P<0.05).The postoperative ROM and HSS were increased(P<0.05),VAS was decreased(P<0.05),the difference being significant in above data between month 3 and month 6,month 3and month 12,month 6 and month 12(P<0.05).The postoperative HKA was increased(P<0.05),FTA was decreased(P<0.05),and with statistically significant difference between month 3 and month 6,month 3 and month 12,month 6 and month 12(P<0.05).Conclusion For middle-aged and elderly patients with KOA,Ilizarov technology combined with rehabilitation training can relieve pain symptoms,improve knee joint activity and knee joint stability,optimize MAL,and promote the recovery of knee joint function.
