小承气汤合黄芪建中汤灌肠对术后早期炎性肠梗阻大鼠肠黏膜损伤及炎性因子的影响

摘　　要：目的研究小承气汤合黄芪建中汤对术后早期炎性肠梗阻(EPISBO)大鼠肠黏膜损伤和炎性介质的影响。方法将45只SPF级Wistar大鼠按照随机数字表法分为正常组、假手术组、模型组、中药组和肥皂水组,每组9只,采用肠管表面擦拭法建立术后早期炎性肠梗阻模型。造模术后第1天开始给药,中药组灌肠给药量为8.45 g/(kg·d),肥皂水组予等体积0.2%肥皂水灌肠,每日1次,连续1周。干预结束后比较大鼠肠粘连级别;ELISA检测比较二氧化胺酶(DAO)、D-乳酸(D-LA)肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)。结果与正常组比较,模型组肠粘连级别、DAO、D-LA、TNF-α、IL-6、IL-10均升高(P<0.05);与模型组比较,中药组肠粘连级别降低(P<0.05),血清DAO、D-LA、TNF-α、IL-6含量降低(P<0.05),IL-10含量升高(P<0.05)。结论小承气汤合黄芪建中汤能减轻EPISBO大鼠的肠黏膜损伤,缓解肠粘连,降低炎症因子水平从而保护肠道屏障功能。Objective:To observe the effect of Xiaochengqi Decoction plus Huangqi Jianzhong Decoction on intestinal mucosa injury and inflammatory factors in Wistar rats with experimental early postoperative inflammatory small bowel obstruction(EPISBO).Methods:45 Wistar rats were randomly divided into normal group,sham surgery group,model group,traditional Chinese medicine enema group and soapy water enema group according to random number table method,with 9 rats in each group.The EPISBO models were generated by wiping the wall of rat′s small intestine.The administration began on the 1st day after the model was successfully prepared,and the dose of traditional Chinese medicine enema group was 8.45 g/(kg·d);soapy water enema group was given to equal volumes of 0.2%soapy water enema once a day for a week.After the intervention,the levels of intestinal adhesion were observed;the blood levels of diamine oxidase(DAO),D-lactic acid(D-LA),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-10(IL-10)were detected by ELISA.Results:Compared with the normal group,the level of intestinal adhesion,DAO,D-LA,TNF-α,IL-6,IL-10 in the model group were all increased(P<0.05);compared with the model group,the levels of intestinal adhesion were decreased(P<0.05),the levels of DAO,D-LA,TNF-α,and IL-6 were reduced(P<0.05),and the levels of IL-10 were increased in the traditional Chinese medicine enema group.Conclusion:Xiaochengqi Decoction plus Huangqi Jianzhong Decoction can reduce intestinal mucosal injury of EPISBO,relieve intestinal adhesion,have a protective effect on the intestinal barrier function in EPISBO rats by affecting the expression of inflammatory factors.
