自拟消肿止痛方对距骨软骨损伤术后踝关节肿痛及炎症指标的影响

摘　　要：目的观察自拟消肿止痛方对距骨软骨损伤微骨折治疗术后踝关节肿痛及相关炎症指标的影响。方法将75例距骨软骨损伤并行微骨折术治疗的患者随机分为观察组38例与对照组37例。对照组术后予常规抗感染、消肿、止痛和间断冷敷治疗,观察组在对照组基础上联合自拟消肿止痛方口服治疗。观察两组治疗后踝关节疼痛程度、肿胀程度、相关炎症指标及药物不良反应。结果术后第3天开始两组患者VAS疼痛评分较术后第1天均开始下降,且观察组较对照组下降明显(P<0.05);术后第3天开始两组患者踝关节肿胀增量值较术后第1天均开始下降,且术后第5、7天观察组较对照组下降明显(P<0.05);术后第7天两组C反应蛋白、肿瘤坏死因子-α、白细胞介素-1β值较术后第1天均明显下降,且观察组较对照组下降明显(P<0.05)。结论自拟消肿止痛方不仅能有效缓解距骨软骨损伤微骨折治疗术后疼痛和促进关节肿胀消退,而且可有效降低相关炎症指标。Objective:To observe the effect of self-made Xiaozhong Zhitong Decoction on the swelling and pain and inflammatory indexes of ankle joint after microfracture surgery in the treatment of talus cartilage injury.Methods:A total of 75 patients with talus cartilage injury and treated by microfracture surgery were randomly divided into the observation group(38 cases)and the control group(37 cases).The control group was treated with routine anti-infection,swelling elimination,pain relief and intermittent cold compress,and the observation group was treated with self-made Xiaozhong Zhitong Decoction on the basis of the control group.The degree of ankle pain,swelling,related inflammatory indexes and adverse drug reactions were observed in the two groups after treatment.Results:The VAS pain scores of the two groups from the third day after operation began to decrease compared with those from the first day after operation,and the decrease in the observation group was more significant than that in the control group(P<0.05).The increment of ankle swelling in the two groups began to decrease from the third day after operation compared with the first day after operation,and the decrease in the observation group was more obvious than that in the control group on the fifth and seventh days after operation(P<0.05).The levels of CRP,TNF-αand IL-1βin the two groups at 7 days after operation were significantly lower than those at 1 day after operation,especially in the observation group(P<0.05).Conclusion:Self-made Xiaozhong Zhitong Decoction can not only effectively relieve postoperative pain and promote joint swelling subsided after microfracture surgery in the treatment of talus cartilage injury,but also effectively reduce related inflammatory indexes.
