防风通圣汤加减联合火针治疗慢性湿疹临床疗效及对全血EOS、血清IL-4、IgE水平的影响

摘　　要：目的:观察火针联合防风通圣汤加减治疗慢性湿疹的临床疗效及其对全血EOS、血清IL-4、IgE水平的影响。方法:将符合纳入标准的72例慢性湿疹患者随机分为两组各36例。对照组予曲安奈德益康唑乳膏外涂患处,每日2次;治疗组行火针基础操作后,对患者行辨证论治,予防风通圣汤加减,两组患者均治疗2周。观察并比较两组疗效。结果:治疗组总有效率为91.6%(33/36),高于对照组的63.8%(23/36),差异有统计学意义(P<0.001);治疗后,两组患者EASI和VAS评分以及外周全血EOS水平,血清IL-4、IgE水平均较治疗前降低,且治疗组的各指标改善情况均优于对照组(P<0.05)。结论:火针联合防风通圣汤加减可以有效改善慢性湿疹患者临床体征,降低外周全血EOS水平及血清IL-4、IgE水平。Objective:To observe the clinical efficacy of fire needling and modified Fangfeng Tongsheng decoction on chronic eczema and its effect on levels whole blood EOS,serum IL-4 and IgE.Methods:72 cases of patients with chronic eczema,who met the inclusion criteria,were randomly divided into two groups,36 cases in each group.The control group was treated with triamcinolone acetonide and econazole nitrate cream which was externally applied on affected area,twice a day;while the treatment group was treated at the first by basic operation of fire needling,then the patients were treated plus with modified Fangfeng Tongsheng decoction according to syndrome differentiation.Both groups were treated for 2 weeks.The clinical efficacy was observed and compared between two groups.Results:After treatment,the total effective rate of treatment group was 91.6%(33/36),which was higher than 63.8%(23/36)of control group,and the difference was statistically significant(P<0.001).After treatment in two groups,the scores of EASI and VAS and the levels of EOS in peripheral whole blood,IL-4 and IgE in serum were decreased compared with before treatment,and the improvement of each index in treatment group was better than that in control group(P<0.05).Conclusion:Fire needling and modified Fangfeng Tongsheng decoction can effectively improve the clinical physical signs of patients with chronic eczema,and reduce the levels of EOS in peripheral whole blood,IL-4 and IgE in serum.
