摘　　要：目的通过对247份HIV-1抗体不确定样本的病毒载量检测结果进行分析,探讨病毒载量检测在不确定样本感染判定中的应用价值。方法采用酶联免疫法和胶体金/硒法对247份HIV-1抗体不确定样本进行筛查试验,然后对筛查试验阳性的样本进行蛋白印迹(WB)检测,对WB检测结果不确定的样本进行病毒载量检测。结果55.47%的不确定样本病毒载量>5000 copies/mL,不确定样本中病毒载量值低于检测线(LDL)的多为女性,而病毒载量>5000 copies/mL的多为男性。通过对WB带型分析发现,gp160 p24带型是判定感染的重要带型。45份抗体阳转样本均能检测到病毒载量值,最低值为740 copies/mL,阳转中位数时间为41 d。91.50%的阳转患者年龄<47岁。结论对于HIV-1抗体不确定样本,应结合流行病学史,参考性别、年龄等因素,在筛查试验阳性基础上,及时采取病毒载量检测判定HIV感染情况。Objective To analyze the viral load test results of 247 HIV-1 antibody indeterminate samples and evaluate the value of viral load test in follow-up diagnosis of HIV-indeterminate cases.Methods Enzyme-linked immunosorbent assay(ELISE)and Rapid test(RT)were used to screen 247 samples,the positive samples were tested by WBtest,and the samples with uncertain WBtest results were tested for viral load.Results 55.47%undetermined samples had virus load>5000 copies/mL.Most samples had virus load below the detection line(LDL)were female,while most samples having virus load>5000 copies/mL were male.Gp160 p24 were important belt types.45 positive samples were able to detect the viral load.The lowest value was 740 copies/ml,and the median time of the antibody became positive was 41days.91.50%positive samples were younger than 47 years old.Conclusion The infection of uncertain HIV-1 antibody samples should be determined according to the epidemiological history,sex,age and other factors.On the basis of positive screening test,virus loading test should be used to determine HIV infection situation.
