摘　　要：目的探讨青少年强迫症脑结构改变的脑区及异常脑区灰质体积与强迫症状的关系。方法对30例青少年强迫症患者与30例健康志愿者进行3.0T磁共振扫描,扫描序列为3D-T_(1)WI,然后对两组数据进行VBM处理分析,对差异脑区的灰质体积值与Y-BOCS总分作相关分析。结果病例组灰质体积降低脑区:左侧颞下回;灰质体积增高脑区:左侧中央前回、右内侧额上回;病例组左侧中央前回灰质体积值与Y-BOCS总分呈Pearson正相关(r=0.866;P=0.042)。结论青少年强迫症存在多个脑区结构异常,且异常脑区均位于大脑皮层,与经典的CSTC环路基本相符合;左侧中央前回灰质体积的改变可能对强迫症状具有重要意义。Objective To investigate brain regions whose structure change in adolescents with obsessive-compulsive disorder and correlation between the gray matter volume(GMV)of abnormal brain regions and obsessive-compulsive symptoms.Methods Thirty adolescent patients with obsessive-compulsive disorder and thirty healthy volunteers will underwent 3.0T magnetic resonance(MRI)scans with 3D-T_(1)WI sequence.Then,Voxel-based morphometry(VBM)analysis was performed for the two groups data and the correlation between the GMV values of different brain regions and the total score of Y-BOCS was analyzed.Results Compared with healthy controls,patients with obsessive-compulsive disorder showed decreased GMV in the left inferior temporal gyrus and increased in the left precentral gyrus,and right medial superior frontal gyrus.Pearson’s correlation analyses show GMV values of the left precentral gyrus are positively correlated with the Y-BOCS total scores in the case group(r=0.866;P=0.042).Conclusion There are some structural abnormalities in some brain regions of adolescent obsessive-compulsive disorder,what’s more,the abnormal brain areas are all in the cerebral cortex,they are consistent with classical CSTC circuit.Change in the GMV of the left precentral gyrus may be important for obsessive-compulsive symptoms.
