糖尿病肾病与非糖尿病肾病血液透析患者低血糖恐惧与抑郁的比较研究

摘　　要：目的描述糖尿病肾病与非糖尿病肾病血液透析患者低血糖发生率,探讨糖尿病肾病与非糖尿病肾病血液透析患者低血糖恐惧程度、抑郁程度是否存在差异。方法选取血液透析患者138例,其中糖尿病肾病患者69例,非糖尿病肾病患者69例。采用低血糖恐惧量表(FH-15)量表、流调中心抑郁量表(CES-D)测量患者的低血糖恐惧和抑郁程度,比较2组患者差异。结果糖尿病肾病组患者低血糖发生率以及FH-15得分均高于非糖尿病肾病组患者,差异有统计学意义(P<0.05)。2组患者CES-D得分差异无统计学意义(P>0.05)。结论糖尿病肾病血液透析患者整体上较非糖尿病肾病血液透析患者更容易发生透析低血糖并且更容易出现低血糖恐惧。建议在为糖尿病肾病血液透析患者进行护理时应考虑此类患者的特点。Objective To understand the incidence of hypoglycemia in patients with diabetic nephropathy(DN)or non-DN during hemodialysis and explore whether there is a difference in the degree of fear of hypoglycemia and depression between DN and non-DN patients on hemodialysis.Methods A total of 138 hemodialysis patients admitted to the Hemodialysis Center of China-Ja⁃pan Friendship Hospital were selected,69 patients with DN and 69 patients with non-DN.The fear of hypoglycemia(FH-15)and the Center for Epidemiological Studies-Depression(CES-D)were used to measure the patients'fear of hypoglycemia,depression,and the differences between the two groups were compared.Results The incidence of hypoglycemia and FH-15 scores in DN group were higher than those in non-DN group(P<0.05),and there was no difference in CES-D scores between the two groups(P>0.05).Conclusion DN group patients are generally more likely to have dialysis hypoglycemia and more likely to have hypoglycemia fear than non-DN group patients.Therefore,it is recommended that the characteristics of such patients should be considered when nursing DN patients on hemodialysis.
