摘　　要：目的:总结老年危重病人谵妄预防的最佳证据,为各级医院在重症监护室(ICU)老年谵妄预防方面提供借鉴与参考。方法:应用复旦大学在循证护理中心开发的PIPOST工具确立初始问题,以“6S”证据模型为指导,逐层确定检索的相关数据库中近10年关于老年危重病人谵妄预防及管理的证据,并对符合方法学质量评价的文献进行证据的提取与汇总。结果:纳入文献14篇,形成谵妄的管理和培训、谵妄的评估、纠正危险因素及谵妄措施的监测4个证据大类,包含17个证据主题的最佳证据总结。结论:该研究汇总了老年危重病人谵妄预防的最佳证据,医务人员应选择针对性的最佳证据,规范老年危重病人谵妄的管理,提高护理质量。Objective:To summarize the best evidence for delirium prevention in elderly critical patients,in order to provide reference for ICU elderly delirium prevention in hospitals at all levels.Methods:The initial questions were established by using the PIPOST tool developed in the Evidence-Based Nursing Center by Fudan University.Under the guidance of the"6S"evidence model,the evidence of delirium prevention and management of elderly critical patients in the relevant database retrieved in the past ten years was determined layer by layer,and the evidence was extracted and summarized according to the literature of methodological quality evaluation.Results:A total of 14 papers were included.The best evidence consisted of four aspects containing management and training of developing delirium,assessment of delirium,correction of risk factors,and delirium monitoring,containing the best evidence summaries for 17 evidence topics.Conclusions:This study summarized the best evidence for the prevention of delirium in elderly critical patients.Medical staff should select the best evidence for pertinence,standardize the management of delirium in elderly critical patients,and improve the quality of nursing.
