摘  要:目的了解唐山地区健康人群维生素D水平及其影响因素,为本地区人群合理补充维生素D及临床治疗维生素D缺乏症提供参考。方法选取在开滦总医院进行体检的唐山各企业员工及本地居民自发查体人员为研究对象,采用化学发光法检测其血清25-羟维生素D(25-hydroxyvitamin D,25OHD)浓度,评价人体维生素D水平。结果唐山地区健康人群血清25OHD平均浓度为(46.63±18.23)nmol/L,缺乏率达52.96%。女性25OHD浓度明显低于男性[(42.05±13.13)nmol/L vs.(47.88±18.38)nmol/L,t=26.512,P<0.01]。随年龄增长血清25OHD浓度逐渐增高,各年龄组血清25OHD浓度比较,差异有统学计意义(F=99.409,P<0.01)。多元Logistic回归分析显示年龄、户外活动时间、饮食中添加蛋奶海鲜肉类、使用保健品(不含维生素D或其他类似物)及防晒护肤用品是维生素D浓度充足与否的相关因素,维生素D浓度充足与年龄增长、饮食中经常添加蛋奶海鲜肉类及使用保健品(不含维生素D或其他类似物)呈正相关(B=2.565、0.396、0.824,P<0.01),而与较少户外活动时间和经常使用防晒护肤品负相关(B=-1.227、-1.817,P<0.01)。结论唐山地区健康人群维生素D浓度处于缺乏水平,年龄、户外活动时间、饮食中添加蛋奶海鲜肉类、使用保健品(不含维生素D或其他类似物)及防晒护肤用品是影响维生素D浓度的相关因素。Objective To investigate the levels of vitamin D and its influencing factors in the healthy population in Tangshan area,providing the reference for rational vitamin D supplementation and clinical treatment of vitamin D deficiency in the healthy population.Methods We recruited the employees of companies and residents for the health examination at the Kailuan General Hospital.Serum 25-hydroxyvitamin D(25OHD)was detected with chemiluminescence.Results The mean serum 25OHD concentration of the healthy population was(46.63±18.23)nmol/L,the vitamin D deficiency rate was 52.96%in Tangshan area.The serum 25OHD concentration in the women[(42.05±13.13)nmol/L]was significantly lower than that of the men[(47.88±18.38)nmol/L](t=26.512,P<0.01).The serum 25OHD concentrations gradually increased with age(F=99.409,P<0.01).Multiple Logistic regression analysis,it was showed that age,outdoor time,dietary supplementation with eating eggs or milk or seafood or meat,using health products(without vitamin D or other analogues),and using sunscreen products were factors affecting the concentration of vitamin D.The sufficient vitamin D concentration was positively correlated with the increasing age,frequent addition of eating eggs or milk or seafood or meat to their diet,and using health products(without vitamin D or other analogues)(B=2.565,0.396,0.824,P<0.01).However,less outdoor time and frequently using sunscreen products were negatively correlated to 25OHD level(B=-1.227,-1.817,P<0.01).Conclusion The vitamin D concentration of the healthy population in Tangshan area is at the levels of vitamin D deficiency,the age,outdoor time,dietary supplementation with eating eggs,milk,seafood,or meat,and using of health products(without vitamin D or other analogues),and using of sunscreen products are associated with vitamin D concentration.
