曲美他嗪联合复方丹参片治疗冠心病合并2型糖尿病患者疗效及其心肌保护机制

摘　　要：目的:探讨曲美他嗪联合复方丹参片治疗冠心病合并2型糖尿病的疗效及其心肌保护机制。方法:选取2020年3月至2022年3月医院收治的70例冠心病合并2型糖尿病患者作为研究对象,随机分为对照组和观察组,各35例。对照组在常规治疗上加用曲美他嗪片,观察组在对照组基础上联合复方丹参片,比较两组临床疗效、治疗前后脂蛋白a(LPa)、同型半胱氨酸(Hcy)、尿酸(UA)和凝血功能[活化部分凝血活酶时间(APPT)、纤维蛋白原(FIB)]水平及不良反应发生情况。结果:观察组总有效率94.29%高于对照组的77.14%(P<0.05);两组治疗后LPa、Hcy、UA、APPT及FIB水平均优于治疗前,且观察组治疗后LPa、Hcy、UA水平均低于对照组,APPT长于对照组,FIB低于对照组(P<0.05);观察组不良反应发生率2.86%低于对照组的17.14%(P<0.05)。结论:曲美他嗪联合复方丹参片治疗冠心病合并2型糖尿病可提高临床疗效,降低LPa、Hcy、UA水平以及改善凝血功能,保护心肌,且不良反应发生率低。Objective:To investigate the therapeutic effect and myocardial protective mechanism of trimetazidine combined with compound Danshen tablets treatment coronary heart disease complicated with type 2 diabetes.Methods:70Patients with coronary heart disease and type 2 diabetes admitted to hospital from March 2020 to March 2022 were selected as the study subjects and randomly divided into two groups,35 cases each.The control group was treated with trimetazidine tablets in addition to the conventional treatment,and the observation group was treated with compound Danshen tablets on the basis of the control group.The clinical efficacy,levels of lipoprotein a(LPa),homocysteine(Hcy),uric acid(UA)and coagulation function[activated partial thromboplastin time(APTT),fibrinogen(FIB)],and adverse reactions of the two groups were compared before and after treatment.Results:The total effective rate of the observation group(94.29%)was higher than that of the control group(77.14%),P<0.05.After treatment,levels of LPa,Hcy,UA,APPT and FIB in both groups were better than those before treatment(P<0.05),and level of LPa,Hcy,UA and FIB in the observation group were lower than those in the control group,APTT was longer than that in the control group(P<0.05);and the total incidence of adverse reactions(2.86%)was lower than that of the control group(17.14%),P<0.05.Conclusion:Trimetazidine combined with compound Danshen tablets in the treatment of coronary heart disease complicated with type 2 diabetes can improve the clinical efficacy;reduce the levels of LPa,Hcy,UA;improve blood coagulation function;protect the myocardium,and the incidence of adverse reactions is low.
