摘　　要：目的探讨加减柴胡温经汤治疗围绝经期肝郁阴虚夹瘀型失眠的临床效果。方法按照随机数表法将96例围绝经期肝郁阴虚夹瘀型失眠患者分成对照组与观察组,各48例。对照组服用坤泰胶囊治疗,观察组予加减柴胡温经汤治疗,2组均治疗4周。观察2组治疗前后中医证候积分、匹兹堡睡眠质量指数(PSQI)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、血清炎性细胞因子白介素-6(IL-6)、白介素-1β(IL-1β)水平改善状况,比较2组治疗总有效率和安全性。结果观察组治疗后的中医证候积分、PSQI评分、SAS评分、SDS评分、IL-6、IL-1β水平均低于对照组(P<0.05);观察组总有效率(93.75%,45/48)高于对照组(81.25%,39/48)(P<0.05);2组不良反应发生率比较,差异无统计学意义(P<0.05)。结论加减柴胡温经汤治疗围绝经期肝郁阴虚夹瘀型失眠患者疗效显著,能够有效调节IL-6、IL-1β水平,改善睡眠质量和负性情绪,且不良反应少。Objective To explore the clinical effect of modified Chaihu Wenjing Decoction in the treatment of perimenopausal insomnia with the syndrome of stagnation of liver qi and yin deficiency complicated with blood stasis.Methods A total of 96 perimenopausal insomnia patients with the syndrome of stagnation of liver qi and yin deficiency complicated with blood stasis were randomly divided into a control group and an observation group.The control group was treated with Kuntai Capsules,while the observation group was treated with modified Chaihu Wenjing Decoction,both for 4 weeks.The improvement in the TCM syndrome score,Pittsburgh Sleep Quality Index(PSQI)score,Self-Rating Anxiety Scale(SAS)score,Self-Rating Depression Scale(SDS)score,serum inflammatory cytokine interleukin-6(IL-6),interleukin-1β(IL-1β)was observed before and after treatment in the two groups.The total effective rate and safety of the two groups were compared.Results The levels of the TCM syndrome score,PSQI score,SAS score,SDS score,IL-6,IL-1βin the observation group were lower than those in the control group after treatment(P<0.05).The total effective rate of the observation group(93.75%,45/48)was higher than that of the control group(81.25%,39/48)(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P<0.05).Conclusion Modified Chaihu Wenjing Decoction is effective in the treatment of perimenopausal insomnia patients with the syndrome of stagnation of liver qi and yin deficiency complicated with blood stasis,which can effectively regulate the levels of IL-6 and IL-1β,improve the patients’sleep quality and negative emotions,with fewer adverse reactions.
