雷火灸对老年骨折下肢术后肢体肿胀患者疼痛及功能恢复的影响

摘　　要：目的分析雷火灸对老年下肢骨折术后肢体肿胀患者疼痛及功能恢复的影响。方法回顾性分析2020年1月—2021年4月广东省中医院就诊的112例老年骨折术后气滞血瘀型患者的临床资料。根据术后治疗方法分为观察组和对照组,各56例。对照组接受骨折术后常规护理及治疗,观察组在对照组的基础上接受雷火灸治疗。比较2组术后14 d临床疗效;比较2组术后7 d及14 d的患肢疼痛状况、肢体肿胀程度;记录2组治疗期间褥疮、感染、尿潴留等并发症发生情况;比较2组术后1个月及3个月的肢体运动能力(Fugl-Meyer下肢运动功能评定量表)。结果术后14 d观察组的总有效率96.43%,与对照组85.71%比较,差异有统计学意义(P<0.05);观察组术后7 d、14 d疼痛情况VAS评分显著低于对照组(P<0.05),患肢肿胀程度较对照组明显改善(P<0.05);观察组术后1个月时的FM下肢运动功能评分显著高于对照组(P<0.05)。结论给予老年下肢骨折患者术后雷火灸干预,可有效活血化瘀、镇痛消肿,促进患肢运动功能的恢复。Objective To analyze the effects of thunder-fire moxibustion on pains and functional recovery in elderly patients with limb swelling after a lower limb fracture surgery.Methods The clinical data of 112 elderly patients with the pattern of qi stagnation and blood stasis after a fracture surgery from January 2020 to April 2021 were analyzed retrospectively.According to the postoperative treatment methods,they were divided into an observation group and a control group,with 56 cases in each group.The control group received a routine nursing and treatment after surgery,while the observation group was treated with thunder-fire moxibustion on the basis of the control group.The clinical effects of the two groups were compared 14 days after operation.The pain status and swelling degree of affected limbs on the 7th and 14th day after operation were compared between the two groups.The occurrence of complications such as pressure sores,infection and urinary retention were recorded during treatment in the two groups.The limb motor ability(Fugl-Meyer Lower Limb Motor Assessment)was compared between the two groups one month and three months after surgery.Results The total effective rate of the observation group(96.43%)was significantly improved compared with that in the control group(85.71%)14 days after operation(P<0.05).The VAS score of pain on the 7th and 14th day after operation in the observation group was significantly lower than that in the control group(P<0.05).The swelling degree of the affected limb on the 7th and 14th day after operation was significantly improved in the observation group compared to the control group(P<0.05).The FM lower limb motor function score in the observation group one month after surgery was significantly higher than that in the control group(P<0.05).Conclusion The intervention with thunder-fire moxibustion for elderly patients with lower limb fractures after surgery can effectively promote blood circulation for removing blood stasis,relieve pain and swelling,and promote the recovery of motor
