摘 要:目的 研究血浆内皮素(PET-1)、尿N-乙酰-B-D氨基葡萄糖苷酶(NAG)、尿激酶型纤溶酶原激活剂受体(uPAR)在糖尿病合并原发性肾病综合征表达及与肾功能的相关性。方法 回顾性分析该院2017年1月—2020年12月该院收治的98例糖尿病合并原发性肾病综合征患者的临床资料,并将其归为研究组;选取同期本院收治的60例原发性肾病综合征及75例2型糖尿病(T2DM)患者分别作为原发性肾病综合征组与T2DM组,另选取在本院进行健康体检的84健康志愿者设为对照组。对比四组PET-1、NAG、uPAR的表达水平;对四组组肾功能[血尿素氮(BUN)、血肌酐(Ser)、尿酸(UA)水平]水平;分析PET-1、NAG、uPAR与糖尿病合并原发性肾病综合征患者肾功能的相关性。结果 研究组PET-1、NAG、uPAR表达水平均明显高于原发性肾病综合征组、T2DM组及对照组(P<0.05);研究组BUN、Ser、UA水均明显高于原发性肾病综合征组、T2DM组及对照组(P<0.05);Spearman相关分析显示,PET-1、NAG、uPAR与糖尿病合并原发性肾病综合征患者BUN、Ser、UA均呈正相关(P<0.05)。结论 糖尿病合并原发性肾病综合征患者PET-1、NAG及uPAR水平较健康者显著升高,且与患者肾功能密切相关,可作为临床诊断糖尿病合并原发性肾病综合征和评估病情的辅助检查。Objective To study the expression of plasma endothelin(PET-1),urine N-acetyl-BD glucosaminidase(NAG),Urokinase type plasminogen activator receptor(uPAR) in diabetes with primary nephrotic syndrome and its correlation with renal function.Methods The clinical data of ninety eight patients with diabetes and primary nephrotic syndrome admitted to our hospital from January 2017 to December 2020 were retrospectively analyzed,and they were classified into the study group;60 patients with primary nephrotic syndrome and 75 patients with type 2 diabetes mellitus(T2 DM) who were treated in our hospital during the same period were selected as the primary nephrotic syndrome group and the T2 DM group,respectively,another 84 healthy volunteers underwent physical examination in the hospital during the same period were set as the control group.Compared the expression levels of PET-1,NAG,uPAR in the four groups;compared the levels of renal function[Blood urea nitrogen(BUN),serum creatinine(Ser),and Uric acid(UA) levels] in the four groups;analyzed the correlation between PET-1,NAG,uPAR and renal function in patients with diabetes and primary nephrotic syndrome.Results The expression levels of PET-1,NAG and uPAR in the study group were significantly higher than those in the primary nephrotic syndrome group,T2 DM group and control group(P<0.05);The levels of BUN,Ser,and UA in the study group were significantly higher than those in the primary nephrotic syndrome group,T2 DM group and control group(P<0.05);Spearman correlation Analysis showed that PET-1,NAG,uPAR were positively correlated with BUN,Ser,and UA in patients with diabetes and primary nephrotic syndrome(P<0.05).Conclusion The levels of PET-1,NAG and uPAR in diabeticpatients with primary nephrotic syndrome are significantly higher than those in healthy patients,and are closely related to the renal function of patients.They can be used as an aid for clinical diagnosis of diabetes with primary nephrotic syndrome and assessment of the condition examine.
