摘 要:目的分析血清25-羟维生素D_(3)[25-hydroxy vitamin D_(3),25-(OH)D_(3)]在糖尿病(diabetes mellitus,DM)中的变化,及其与糖尿病大血管并发症(macro-vascular complications,MVC)的相关性。方法选取2018年2月—2019年6月宁德师范学院附属宁德市医院收治的120例DM患者作为研究对象。以血清25-(OH)D_(3)水平分为缺乏组、不足组及正常组,分析血清25-(OH)D_(3)水平与DM一般资料的相关性,并以是否发生MVC为依据,分析血清25-(OH)D_(3)的预测价值。结果25-(OH)D_(3)不同浓度的HbA1c、TG、TC、FPG、hs-CRP水平及MVC发生率比较,差异有统计学意义(P<0.05)。HbA1c、TG、TC、FPG及hs-CRP与25-(OH)D_(3)均呈相关性(P<0.05)。发生MVC患者血清25-(OH)D_(3)水平为(18.36±9.69)ng/mL,显著低于无MVC患者,差异有统计学意义(t=-6.402,P<0.05)。ROC曲线结果显示,25-(OH)D_(3)取24.27 ng/mL时,对DM合并MVC的预测效能最高,AUC为0.821(95%CI:0.746~0.895,P<0.001)、敏感度为0.717、特异度为0.833。结论25-(OH)D_(3)缺乏则MVC发生风险越高,且25-(OH)D_(3)可用于预测MVC。Objective To analyze the changes of serum 25-hydroxy vitamin D_(3)[25-(OH)D_(3)]in diabetes mellitus(DM)and its correlation with macro-vascular complications(MVC)in diabetes mellitus(DM).Methods 120 patients with DM admitted to Ningde Hospital Affiliated to Ningde Normal University from February 2018 to June 2019 were selected as the study population.Serum 25-(OH)D_(3)levels were used to divide into deficiency group,insufficient group and normal group.The correlation between serum 25-(OH)D_(3)level and general information of DM was analyzed,and the predictive value of serum 25-(OH)D_(3)was analyzed based on whether MVC occurred or not.Results There were significant differences in the levels of HbA1c,TG,TC,FPG,hs-CRP and the incidence of MVC among different concentrations of 25-(OH)D_(3),the difference was statistically significant(P<0.05).HbA1c,TG,TC,FPG and hs-CRP were correlated with 25-(OH)D_(3)(P<0.05).Serum 25-(OH)D_(3)in patients with MVC was(18.36±9.69)ng/mL significantly lower than that in patients without MVC,the difference was statistically significant(t=-6.402,P<0.05).The results of ROC curve showed that when 25-(OH)D_(3)was 24.27 ng/mL,the predictive efficacy of DM combined with MVC was the highest,with AUC of 0.821(95%CI:0.746-0.895,P<0.001),sensitivity of 0.717 and specificity of 0.833.Conclusion 25-(OH)D_(3)deficiency is associated with a higher risk of MVC,and 25-(OH)D_(3)deficiency can be used to predict MVC.
