达格列净联合胰岛素对2型糖尿病早期肾病患者尿微量白蛋白的影响研究

摘　　要：目的分析2型糖尿病合并早期肾病患者经胰岛素联合达格列净治疗对血糖、血脂、肾功能的影响。方法选取2020年5月—2022年5月于苏州市相城人民医院接受治疗的2型糖尿病合并早期肾病患者224例为研究对象,利用计算机随机抽号方式将患者分为对照组(112例)与观察组(112例),对照组单纯采用胰岛素治疗,观察组在其基础上联用达格列净,比较两组患者治疗前后血糖指标、血脂指标差异,测定两组患者治疗前及用药1周、2周、4周尿微量白蛋白指标变化。观察分析两组患者治疗期间出现不良反应的总发生率。结果治疗后,观察组血糖指标、血脂指标水平均低于对照组,差异有统计学意义(P<0.05);观察组患者用药1~4周后尿微量白蛋白指标均低于对照组,差异有统计学意义(P<0.05)。观察组患者不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论2型糖尿病合并早期肾病患者在胰岛素治疗基础上联用达格列净可在稳定降糖的基础上改善患者肾功能,调节尿微量白蛋白水平,同时不良反应少。Objective To analyze the effects of insulin combined with Dapagliflozin on blood glucose,blood lipids and renal function in patients with type 2 diabetes mellitus complicated with early nephropathy.Methods 224 patients with type 2 diabetes and early nephropathy who were treated in Suzhou Xiangcheng People’s Hospital from May 2020 to May 2022 were selected as the study subjects.The patients were randomly divided into control group(112 cases)and observation group(112 cases).The control group was treated with insulin alone,and the observation group was treated with Dapagliflozin on the basis of this.The differences of blood glucose index and blood lipid index were compared between the two groups before and after treatment,and the changes of urinary microalbumin index were measured between the two groups before treatment and after 1,2 and 4 weeks of medication.The total incidence of adverse reactions in the two groups during treatment were observed and analyzed.Results After treatment,the levels of blood glucose and blood lipid in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The urinary microalbumin indexes of the patients in the observation group were lower than those in the control group after 1~4 weeks of medication,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion In the patients with type 2 diabetes mellitus combined with early nephropathy,the combination of Dapagliflozin on the basis of insulin therapy can improve the patients’renal function and regulate the level of urinary microalbumin on the basis of stable hypoglycemia,and at the same time,the adverse reactions are less.
