摘　　要：目的:探究联合使用氨溴索、茶碱缓释片治疗慢性阻塞性肺疾病急性加重期(AECOPD)并呼吸衰竭老年患者的临床效果。方法:选取本院2020年1月—2022年1月诊治的AECOPD并呼吸衰竭老年患者80例为研究对象,随机分为研究组(40例)和对照组(40例),分别使用氨溴索、联合茶碱缓释片治疗。比较两组治疗前后血气状态变化[血氧分压(PaO_(2))、血氧饱和度(SaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、肺功能指标变化[第1秒用力呼气的容积占预计值的百分比(FEV_(1)%Pred)、用力肺活量(Forced vital capacity,FVC)、呼吸气流变化率(PFF)]以及治疗后的不良反应发生率。结果:两组治疗后PaO_(2)、SaO_(2)高于治疗前,PaCO_(2)低于治疗前,且研究组优于对照组(P<0.05);两组治疗后FEV_(1)%Pred、FVC、PFF水平均高于治疗前,且研究组高于对照组(P<0.05);研究组不良反应发生率为7.50%,低于对照组的25.00%(P<0.05)。结论:联合使用氨溴索、茶碱缓释片治疗AECOPD并呼吸衰竭老年患者具有显著疗效,可有效改善患者血气状态,提高肺功能相关指标,且安全性高,值得临床应用。Objective: To explore the clinical effect of combined use of ambroxol and theophylline sustained-release tablets in the treatment of elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and respiratory failure. Methods: A total of 80 elderly patients with AECOPD and respiratory failure diagnosed and treated in our hospital from January 2020 to January 2022 were randomly divided into study group(40 cases) and control group(40cases), treated with ambroxol and combined with theophylline sustained-release tablets respectively. The changes of blood gas status [oxygen pressure(PaO_(2)), oxygen saturation(SaO_(2)), arterial carbon dioxide pressure(PaCO_(2))], pulmonary function index change [1 second forced expiration volume of the expected value(FEV_(1)%Pred), forced pulmonary capacity(Forced vital capacity, FVC), respiratory airflow change rate(PFF)] and the incidence of adverse reactions after treatment were compared between the two groups before and after treatment. Results: After treatment, PaO_(2)and SaO_(2)in the two groups were higher than those before treatment, PaCO_(2)was lower than that before treatment, and the study group was better than the control group(P<0.05). The levels of FEV_(1)%Pred, FVC and PFF in the two groups after treatment were higher than those before treatment, and the study group was higher than the control group(P<0.05). The incidence of adverse reactions in the study group was 7.50%, which was lower than 25.00% in the control group(P<0.05). Conclusion: Ambroxol combined with theophylline sustained-release tablets in the treatment of AECOPD and respiratory failure in elderly patients with significant effect, which can effectively improve the patient’s blood gas status, improve lung function-related indicators, with high safety,and is worthy of clinical application.
