摘 要:目的:观察256层螺旋CT血管造影(CTA)在小儿复杂先天性心脏病(CCDH)中的诊断效能。方法:回顾性分析2021年1月至2022年1月该院收治的63例CCDH患儿的临床资料,均行CTA检查和经胸超声心动图(TTE)检查,以数字减影血管造影(DSA)检查结果为金标准,比较CTA和TTE检查对CCDH患儿心外结构畸形、心内结构畸形、大血管畸形的检出率。结果:DSA检查结果显示,63例患儿中共检出162处畸形,其中心内结构畸形52处、心外结构畸形32处、大血管畸形78处;TTE检查共检出150处畸形,检出率为92.59%,其中心内结构畸形51处、心外结构畸形29处、大血管畸形70处;CTA检查共检出157处畸形,检出率为96.91%,其中心内结构畸形49处、心外结构畸形30处、大血管畸形78处;TTE与CTA检查的检出率比较,差异无统计学意义(P>0.05);两种检查方法对心内结构、心外结构畸形的检出率比较,差异无统计学意义(P>0.05);CTA检查大血管畸形检出率为100.00%,高于TTE的89.74%,差异有统计学意义(P<0.05)。结论:CTA与TTE检查诊断心内结构畸形、心外结构畸形的检出率相当,但CTA检查对大血管畸形的检出率高于TTE检查。Objective: To observe diagnostic efficacy of 256-slice spiral CT angiography(CTA) in children with complex congenital heart disease(CCDH). Methods: The clinical data of 63 children with CCDH admitted to this hospital from January 2021 to January 2022 were retrospectively analyzed. CTA and transthoracic echocardiography(TTE) were performed. Using the results of digital subtraction angiography(DSA)as the gold standard, the detection rates of CTA and TTE in extracardiac structural malformations, intracardiac structural malformations and large vessel malformations in the children with CCDH were compared. Results: The results of gold standard examination showed 162 malformations in 63 cases, including 52 intracardiac structural malformations, 32 extracardiac structural malformations and 78 macrovascular malformations.A total of 150 malformations were detected by TTE examination, the detection rate was 92.59%, among which 51 were intracardiac structural malformations, 29 were extracardiac structural malformations, and 70 were great vascular malformations. A total of 157 malformations were detected by CTA examination, the detection rate was 96.91%, among which 49 were intracardiac structural malformations, 30 were extracardiac structural malformations, and 78 were great vascular malformations.There was no significant difference in the detection rate between TTE and CTA(P>0.05).There were no significant differences in the detection rates of intracardiac structural malformations and extracardiac structural malformations between the two methods(P>0.05). Further, the detection rate of large vessel malformation by CTA was 100.00%, which was higher than 89.74% by TTE,and the difference was statistically significant(P<0.05). Conclusions: The detection rates of CTA and TTE in the diagnosis of intracardiac structural malformations and extracardiac structural malformations are comparable. However, the detection rate of CTA in the diagnosis of large vessel malformation is higher than that of TTE.
