针刺配合益气通络汤治疗糖尿病周围神经病变临床观察

摘　　要：目的探究针刺配合益气通络汤治疗糖尿病周围神经病变患者的临床效果。方法选取2018年5月—2020年8月江西省九江市第一人民医院接收的80例糖尿病周围神经病变患者作为此次研究对象,并采用随机数字表法将其分为常规组和研究组,各40例。常规组采用常规治疗方案,而研究组采用针刺配合益气通络汤治疗方案。结果治疗后,研究组的临床总有效率为90.00%(36/40),明显高于常规组的70.00%(28/40)(P<0.05);同时,研究组的正中神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)和腓总神经的MNCV、SNCV均明显高于常规组(P<0.05);且研究组的脑源性神经营养因子(BDNF)、胰岛素样生长因子-1(IGF-1)水平明显高于常规组,其髓鞘碱性蛋白(MBP)水平明显低于常规组(P<0.05)。结论针刺配合益气通络汤治疗糖尿病周围神经病变,能有效改善患者的神经传导速度,并调节神经功能因子水平,提高临床治疗效果。Objective To explore the clinical effect of acupuncture combined with Yiqi Tongluo decoction in the treatment of diabetic peripheral neuropathy.Methods 80 patients with diabetic peripheral neuropathy from May 2018 to August 2020 in Jiujiang NO.1 People’s Hospital were selected as the research objects,and they were randomly divided into the conventional group and the study group,with 40 cases in each group.The conventional group was treated with conventional treatment,while the study group was treated with acupuncture and Yiqi Tongluo decoction.Results After treatment,the total clinical effective rate of the study group was 90.00%(36/40),which was significantly higher than that of the conventional group(70.00%(28/40)(P<0.05).Meanwhile,the motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of median nerve and the MNCV and SNCV of common peroneal nerve in study group were significantly higher than those in conventional group(P<0.05).The levels of brain-derived neurotrophic factor(BDNF)and insulin-like growth factor-1(IGF-1)in the study group were significantly higher than those in the conventional group,and the levels of myelin basic protein(MBP)in the study group were significantly lower than those in the conventional group(P<0.05).Conclusion Acupuncture combined with Yiqi Tongluo decoction in the treatment of diabetic peripheral neuropathy can effectively improve the nerve conduction velocity,regulate the level of nerve function factors and improve the clinical therapeutic effect.
