COVID-19流行期间1053名发热门诊患者临床特征回顾性分析

摘　　要：目的分析COVID-19流行期间我院1053名发热门诊患者的临床特征,为发热门诊患者有效诊治提供科学依据。方法采用回顾性分析方法分析2021年5月至9月到我院发热门诊就诊患者的临床资料,包括年龄、性别、体温、发热时间,主要及伴随症状、相关辅助检查等。结果发热门诊就诊患者男性略多于女性;就诊时体温多在39℃以下。45.87%的患者白细胞升高,65.05%的患者中性粒细胞升高。61.54%的患者超敏C反应蛋白增高。16.28%患者胸部CT提示“炎性改变”。发热患者疾病谱依次为:呼吸道感染、胃肠感染、泌尿系统感染、皮肤感染、急性乳腺炎。结论近期发热门诊就诊人数较多,病种涉及多学科。发热门诊应配有具有呼吸道传染病或感染性疾病诊疗经验的医务人员,更有利于为患者提供及时有效的救治及充分发挥发热门诊的早期预警作用。Objective The clinical characteristics of 1053 fever outpatients in our hospital during the COVID-19 epidemic were analyzed to provide a scientific basis for effective diagnosis and treatment of fever outpatients.Methods A retrospective analysis method was used to analyze the clinical data of patients who visited the fever clinic of our hospital from May to September 2021,including age,gender,body temperature,fever time,main and accompanying symptoms,and related auxiliary examinations.Results There were slightly more males than females in the fever clinic;the body temperature was mostly below 39℃.White blood cells were elevated in 45.87%of patients,and neutrophils were elevated in 65.05%of patients.High-sensitivity C-reactive protein was increased in 61.54%of patients.16.28%of patients showed“inflammatory changes”on chest CT.The disease spectrum of febrile patients was:respiratory tract infection,gastrointestinal infection,urinary system infection,skin infection,and acute mastitis.Conclusion There have been a large number of outpatient visits for fever,and the disease types involve multiple disciplines recently.Fever clinics should be equipped with medical staff with experience in the diagnosis and treatment of respiratory infectious diseases or infectious diseases,which is more conducive to providing timely and effective treatment for patients and giving full play to the early warning role of fever clinics.
