药线点灸对血液透析患者动静脉内瘘成熟及并发症的影响

摘　　要：目的探讨壮医药线点灸对血液透析患者动静脉内瘘成熟的临床疗效及并发症的影响。方法选取2021年1—12月南宁市第二人民医院五象医院收治的86例首次行自体动静脉内瘘术患者为研究对象,根据随机数表法分为观察组与对照组。对照组予常规治疗及护理,并于术前2周、术后12 h开始术侧肢体功能锻炼。观察组在对照组的基础上于术前2周、术后24 h应用根据辨证施治组方的壮医药线点灸治疗。比较两组术前2周、术前1天及术后4周、12周头静脉内径,术后4周、12周内瘘静脉血流量,术后4~12周动静脉内瘘成熟情况、成熟时间及6个月内并发症发生率。结果两组患者术前2周头静脉内径比较,差异无统计学意义(P>0.05);观察组术前1天、术后4周、术后12周头静脉内径均大于对照组,差异有统计学意义(P<0.05);术后4周、12周观察组内瘘血流量均大于对照组,差异有统计学意义(P<0.05),观察组内瘘成熟时间短于对照组,内瘘物理成熟率、超声成熟率均高于对照组,术后6个月内并发症发生率低于对照组,差异均有统计学意(P<0.05)。结论壮医药线点灸可改善内瘘术前血管条件,增加血流量、降低炎性反应、促进内瘘成熟及降低内瘘并发症发生率。Objective To explore the effects of medicated thread moxibustion of Zhuang medicine on the clinical efficacy on the maturation of arteriovenous fistulas as well as the complications of arteriovenous fistulas in hemodialysis patients.Methods A total of 86 patients admitted to Wuxiang Hospital,the Second Nanning People’s Hospital from January to December 2021 who underwent autogenous arteriovenous fistula angioplasty for the first time were selected as the study subj ects and divided into the observation group and the c ontrol group according to the random number table method.The control group was given conventional treatment and nursing,and was recommended to exercise the function of the operated limb at 2 weeks before surgery and 12 hours after surgery.Guided by the concept of therapy with syndrome differentiation,the observation group was treated with medicated thread moxibustion of Zhuang medicine at 2 weeks before surgery and 24 hours after surgery on the basis of the treatment in the control group.The two groups were compared for the internal diameter of the cephalic vein at 2 weeks before surgery,1 day before surgery,and 4 weeks and 12 weeks after surgery,the blood flow of the vena with fistula at 4 weeks and 12 weeks after surgery,as well as the maturation of the arteriovenous fistulas from 4 to 12 weeks after surgery,the maturation time and the incidence of complications within 6 months.Results There was no statistically significant difference between the two groups in the internal diameter of the cephalic vein at 2 weeks before surgery(P>0.05),and the internal diameter of the cephalic vein in the observation group was greater than that in the control group at 1 day before surgery,4 weeks and 12 weeks after surgery,with statistically significant difference(P<0.05).The blood flow at the venous end of the observation group was higher than that of the control group at 4 weeks and 12 weeks after surgery,with statistically significant diff erence(P<0.05)..The maturation time of the internal fistulas in the ob
