胃癌术中行洛铂腹腔化疗联合术后早期芪黄煎剂的应用效果

摘　　要：目的探讨洛铂联合芪黄煎剂在胃癌术后早期应用的临床疗效。方法前瞻性纳入2019年1月至2021年12月在安徽中医药大学第一附属医院普外科接受胃癌手术64例患者,按随机数字表法分成对照组和研究组,每组均为32例。对照组术中采用洛铂腹腔化疗,术后常规补液、抗感染、营养支持,术后第1天开始肠内营养管滴注0.9%氯化钠注射溶液和肠内营养制剂;研究组在对照组基础上,术后第1天肠内营养管滴注健脾通里中药芪黄煎剂。比较2组术后胃肠道功能恢复时间,肛门排气、首次排便时间,食欲恢复时间,术后营养恢复情况,血液及肝肾毒性,术后并发症的发生情况。结果研究组胃肠道功能恢复时间(肠鸣音恢复时间、肛门排气及首次排便时间、食欲恢复时间)较对照组明显缩短(P<0.05),研究组术后14 d营养恢复情况较对照组明显改善,差异有统计学意义(P<0.05),2组血液及肝肾毒性、术后并发症的发生率比较,差异无统计学意义(P>0.05)。结论胃癌术中洛铂腹腔化疗,术后早期应用芪黄煎剂肠内营养滴注,能够促进胃肠功能早期恢复,改善患者营养情况,患者耐受性良好。Objective To observe the clinical effect of loplatin combined with Qihuang Decoction on Gastrointestinal in early Application.Methods A total of 64 gastric cancer patients were randomly divided into research group and control group.Each group was 32 gastric cancer patients.The control group received intraperitoneal chemotherapy with loplatin,routine fluid rehydration,anti-infection and nutritional support after operation.The enteral nutrition tube was used to drip physiological saline and enteral nutrition preparations on the first day after the operation.The group was injected with the decoction of Qihuang Decoction of Jianpi Tongli through enteral nutrition tube based on the treatment of the control group.To observe the recovery time of gastrointestinal function and anal exhaust,defecation time,appetite recovery time,postoperative nutrition recovery,blood and liver and kidney toxicity,the incidence of postoperative complications.Results The recovery time of gastrointestinal function in the study group was significantly shorter than that in the control group(P<0.05).After 14 days of treatment,the nutritional levels in the study group significantly improved compared with the control group(P<0.05).There was no significant difference in the incidence of blood,liver and kidney toxicity and postoperative complications between the two groups(P>0.05).Conclusions The early treatment of Qihuang Decoction in the operation of gastric cancer is beneficial to restore gastrointestinal motor function and reduce the incidence of complications,enhance patient tolerance.
