摘　　要：目的促进碳青霉烯类抗菌药物的临床安全、合理应用。方法计算机检索维普、万方、中国知网、PubMed、Embase等数据库自建库起至2022年3月有关碳青霉烯类抗菌药物致血小板增多不良反应的文献,并结合临床药师参与治疗1例美罗培南致血小板增多的病例资料,分析其临床特点及干预对策。结果该例患者血小板的升高、下降与美罗培南存在明显的剂量和时间相关性,停药后未做特殊处理,血小板计数逐渐恢复正常。共检索到国内的个案报道12例,未见国外的相关个案报道。继发性血小板增多症是良性反应,但是否会导致不良后果应结合患者的疾病、合并症、使用药物等综合评估,并制订干预对策。结论碳青霉烯类抗菌药物致血小板增多较少发生,但可能造成严重后果。临床需加强关注,并结合患者的病情采取合适的防范措施;医务人员应加强对碳青霉烯类抗菌药物药品不良反应的认识,增强安全用药意识,促进临床安全、合理用药。Objective To promote the safe and rational use of carbapenems in the clinic.Methods The literature on the adverse drug reactions of thrombocytosis induced by carbapenems in the VIP,WanFang,CNKI,PubMed and Embase databases from the inception to March 2022 was searched,and the clinical characteristics and intervention measures of this disease were analyzed based on the data of a patient with thrombocytosis induced by meropenem treated by clinical pharmacists.Results The increase and decrease of platelets in this patient were significantly related to the dosage and time of meropenem,and the platelet count gradually returned to the normal level after quitting of meropenem without special treatment.A total of 12 domestic case reports were searched,and there was no foreign case reports.Secondary thrombocytosis was a benign reaction,but whether it would induce adverse outcome should be comprehensively evaluated,used drugs and other factors of patients,and intervention measures should be formulated.Conclusion The thrombocytosis induced by carbapenems is a rare adverse reaction,but may induce serious outcome.We should pay more attention and take appropriate preventive measures according to the patients'condition in the clinic.Medical staff should strengthen the understanding for adverse drug reactions induced by carbapenems,and improve the awareness of safe drug use to promote the safe and rational drug use in the clinic.
