摘　　要：报告一家三口同患羊痘。例1.患者男,41岁。有病羊接触史,左手背及右前臂脓疱、结节伴痛痒2周。皮肤科检查:左手背一直径约1 cm乳头瘤样结节;右前臂可见数个直径1~1.5 cm扁平脓疱。皮肤镜检查:镜下可见皮损中央溃疡结痂区及周围黄白色环状区域,可见点状血管。皮损组织病理检查:表皮细胞内及细胞间水肿、空泡形成及气球状变性;真皮血管内皮细胞和表皮细胞胞质可见嗜酸性包涵体。例2和例3患者分别为例1父亲及母亲,皮肤科检查与例1类似。例2发病前有病羊接触史,例3否认发病前有病羊接触史。3例患者结合典型临床表现及皮肤镜、组织病理检查结果,均诊断为羊痘。Three cases of human orf infection within one family are reported.Case 1 was a 41-year-old male,who had a history of contact with diseased sheep,presented with pustules and nodules on the back of his left hand and right forearm for 2 weeks.Physical examination showed a papillomatous nodule,measuring approximately 1 cm in diameter,on the back of the left hand.Several flat pustules of 1-1.5 cm in diameter were also seen on the right forearm.Dermoscopic examination revealed a center ulceration with crust,yellow-white outer halo and dot-like vessels.Histopathological examination showed intracellular and intercellular edema within the epidermis,with vacuolization and ballooning degeneration of the keratinocytes.Intracytoplasmic and intranuclear inclusions were noted within vacuolated keratinocytes as well as endothelial cells of dermal vessels.Case 2 and case 3 were the father and mother of case 1,respectively.Dermatological examination of the case 2 and case 3 showed similar changes as that of case 1.Case 2 had a history of contact with diseased sheep before onset,while case 3 denied the history of contact with diseased sheep before onset.Three patients were diagnosed with orf infection,based on clinical manifestations as well as the results of dermoscopic and histopathological examination.
