A超测量白内障第一眼的屈光误差值对第二眼人工晶体选择的参考意义

摘  要:目的:探究A超测量白内障第一眼的屈光误差值对第二眼人工晶体选择的影响,为临床提供理论参考,以提高白内障的诊疗水平,为更多白内障患者带去福音。方法:选取2020年1月—2021年1月江门市新会区人民医院眼科收治的150例双眼白内障手术患者(总共300眼)作为研究对象,手术前均采用接触式A型超声,对患者眼球的生物学参数进行测量。选择预计屈光度数,术后1个月进行验光,并对屈光误差进行计算,比较双眼眼轴的长度,进行双眼屈光误差分析,探讨两眼屈光误差间的相关性以及第一眼屈光误差的调整对第二眼人工晶体选择的参考意义。结果:在双眼眼轴长度方面,第一眼轴(23.42±1.34)mm,第二眼轴(23.42±1.34)mm。两眼眼轴长度相比较,差异无统计学意义(t=0.247,P>0.05)。双眼屈光误差分布相比较,差异无统计学意义(P>0.05)。双眼屈光误差相关性明显,差异有统计学意义(r=0.812)。20%、50%、80%、100%的调整均可降低第二眼平均绝对屈光误差值,且与调整前相比,差异有统计学意义(P<0.05)。误差在50%以内调整可使所有第二眼绝对屈光误差降低到1.0D内,其中,有84%在0.5D范围内,60.5%在0.25D范围内。按80%及以上的梯度调整系数时,第二眼屈光误差的绝对值全都分布在0.25D范围内,其中,采用80%的梯度调整系数时,第二眼的绝对屈光误差值平均数最小,在0.18D范围内。结论:对于白内障患者而言,经A超测量第一眼屈光误差,若该误差较大时,会对第二眼人工晶体的选择造成较大的影响。本研究提示第二眼人工晶体根据第一眼屈光误差的80%调整进行选择,可将术后的屈光误差绝对值降到最小。Objective:To investigate the influence of refractive error of the first eye of a-super measurement on the choice of intraocu⁃lar lens of the second eye,so as to provide theoretical reference for clinical practice and improve the diagnosis and treatment level of cataract.Methods:150 patients(300 eyes in total)admitted to the hospital ophthalmology department for bilateral cataract surgery from January 2020 to January 2021 were selected for the study.The biological parameters of the patients’eye were measured before surgery using contact A-mode ultrasound in all cases.The expected number of refractive error was selected,and the refractive error was calculated for the postoperative 1 month optometry.The lengths of the axes of both eyes were compared,and a binocular refractive error analysis was performed to explore the correlation between the refractive errors of the two eyes and the reference significance of the adjustment of the refractive error of the first eye for the selection of the second eye IOL.Results:The first eye axis length was(23.42±1.34)mm and the second eye axis length was(23.42±1.34)mm,and the difference between the two eye axis lengths was not statistically significant(t=0.247,P>0.05).There was no statistical difference in the distribution of refractive errors between the eyes(P>0.05).The binocular refractive error correlation was significant and the difference was statistically significant(r=0.812).Adjust⁃ments of 20%,50%,80%,and 100%all reduced the mean absolute refractive error value in the second eye,and the differences were statistically significant compared with those before adjustment(P<0.05).Adjustments within 50%of the error reduced the absolute refractive error to within 1.0D for all second eyes,with 84%within 0.5D and 60.5%within 0.25D.The absolute values of refractive error in the second eye were all distributed within 0.25D when adjusted by gradient adjustment factors of 80%and above,with the smallest mean absolute refractive error value in the second eye within 0.18D when an 8
