摘  要:目的探究miR-197-3p在川崎病患儿血浆中的表达及与冠状动脉病变的相关性。方法于2019-11/2021-11月收集在作者医院接受治疗的急性期川崎病患儿88例(川崎病组)为研究对象。根据冠状动脉损害(coronary artery lesions,CAL)诊断标准分为CAL组(n=29)和冠状动脉未损害组(NCAL组,n=59),选择同期作者医院体检的健康者95例为对照组。实时荧光逆转录定量聚合酶链反应(real-time reverse transcription quantitative polymerase chain reaction,RT-qPCR)检测血浆miR-197-3p水平。采用Pearson法分析miR-197-3p与N末端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)、降钙素原(procalcitonin,PCT)、C反应蛋白(C-reaction protein,CRP)相关性。Logistic回归分析探讨川崎病患儿发生CAL的危险因素。受试者工作特征(receiver operating characteristic,ROC)曲线评价血浆miR-197-3p与NT-proBNP对川崎病患儿发生CAL的预测价值。结果与对照组比较,川崎病组血浆miR-197-3p水平较高(P<0.05)。与NCAL组相比,CAL组血浆miR-197-3p、NT-proBNP、PCT、CRP水平较高(P均<0.05)。Pearson相关分析显示,miR-197-3p与PCT、CRP、NT-proBNP呈正相关(P<0.05)。Logistic回归分析表明,NT-proBNP、miR-197-3p是川崎病患儿发生CAL的危险因素(P<0.05)。ROC曲线分析显示,miR-197-3p预测川崎病患儿发生CAL的曲线下面积(area under the curve,AUC)为0.890(95%CI:0.807~0.973),其灵敏度为89.70%,特异度为91.50%。结论川崎病患儿血浆miR-197-3p水平上调,与CAL发生具有密切关系,血浆miR-197-3p对川崎病患儿发生CAL具有较好的预测价值。Objective To investigate the expression of miR-197-3 p in plasma of children with Kawasaki disease and its correlation with coronary artery disease.Methods A total of 88 children with acute Kawasaki disease(Kawasaki disease group)who were treated in the author′s hospital from November 2019 to November 2021 were collected as research subjects.According to the diagnostic criteria of coronary artery lesions(CAL),they were divided into CAL group(n=29)and coronary artery non-damaged group(NCAL group,n=59),95 healthy people who underwent physical examination in the author′s hospital during the same period were selected as the control group.Plasma miR-197-3 p levels were detected by real-time reverse transcription quantitative polymerase chain reaction(RT-qPCR).The correlation between miR-197-3 p and N-terminal pro-brain natriuretic peptide(NT-proBNP),procalcitonin(PCT),C-reaction protein(CRP)was analyzed by Pearson method.The risk factors of CAL in children with Kawasaki disease were analyzed by Logistic regression analysis.The predictive value of plasma miR-197-3 p and NT-proBNP in children with Kawasaki disease developing CAL was evaluated by receiver operating characteristic(ROC)curve.Results Compared with the control group,the levels of plasma miR-197-3 p in the Kawasaki disease group was higher(P<0.05).Compared with the NCAL group,the levels of plasma miR-197-3 p,NT-proBNP,PCT and CRP in the CAL group were higher(all P<0.05).Pearson correlation analysis showed that miR-197-3 p was positively correlated with PCT,CRP,NT-proBNP(P<0.05).Logistic regression analysis showed that NT-proBNP and miR-197-3 p were risk factors for CAL in children with Kawasaki disease(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)of miR-197-3 p for predicting CAL in children with Kawasaki disease was 0.890(95%CI:0.807-0.973),with the sensitivity of 89.70%and specificity of 91.50%.Conclusion The level of plasma miR-197-3 p in children with Kawasaki disease is up-regulated,and it is closely related to the occurren
