摘　　要：目的:分析2010-2020年公立医院收支结构的变化特点及趋势,为优化公立医院收支结构提供参考。方法:收集2010-2020年公立医院收支数据,采用线图和结构变动度法进行描述性分析。结果:2010-2019年,二、三级公立医院财政补助收入占比分别为9.4%~13.2%和6.2%~8.1%。2012-2020年,药品收入占医疗收入的比例逐年下降,检查与卫生材料收入占医疗收入的比例逐年上升,医疗服务性收入占比增长缓慢。2012-2020年,公立医院医疗收入总体结构变动度为23.54%,医疗收入结构变动贡献从大到小依次为药品收入、卫生材料收入、医疗服务性收入和检查收入。结论:药品零差率等政策的实施总体上促进了公立医院医疗收支结构朝着合理化方向发展,但收支结构仍存在进一步优化空间。Objective:To analyze the characteristics and trends of changes in the revenue and expenditure structure of public hospitals in China from 2010-2020,and to provide a reference basis for optimizing the revenue and expenditure structure of public hospitals.Methods:Data on the revenue and expenditure of public hospitals in China from 2010 to 2020 were collected and descriptive analysis was conducted using line graphs and the degree of structure variation method.Results:From 2010 to 2019,the proportion of financial assistance revenue of GradeⅡand GradeⅢpublic hospitals in China ranged from 9.4%to 13.2%and 6.2%to 8.1%,respectively.From 2012 to 2020,the proportion of drug income to medical income decreased year by year,the balance of examination and health material income to medical income increased year by year,and the ratio of medical service income grew slowly.From 2012 to 2020,the overall structural change in medical income of public hospitals in China was 23.54%,and the contribution of the structural change of medical income was in the order of drug income,health material income,medical service income,and examination income.Conclusions:Since the new health care reform,the implementation of policies such as the zero-markup policy for drugs has generally promoted the rationalization of the medical income and expenditure structure of public hospitals,but there is still room for further optimization of the revenue and expenditure structure.
