摘  要:目的探讨和分析细胞角蛋白19(cytokeratin19,CK19)、人骨髓内皮细胞标记物(mesothelioma,HBME-1)和半乳糖凝集素-3(galectin-3,Gal-3)联合检测在甲状腺乳头状微小癌(papillary thyroid microcarcinoma,PTMC)中的表达及其诊断价值。方法选取2021年10月—2022年9月湖南省职业病防治院甲状腺乳头状微小癌与甲状腺良性乳头状病变患者各48例,采用免疫组化方法检测CK19、HBME-1和Gal-3在两组病变中的表达状况。结果甲状腺乳头状微小癌组CK19、HBME-1和Gal-3阳性率明显高于甲状腺良性病变组,差异有统计学意义(P<0.01);单项检测中CK19和Gal-3灵敏度最高,HBME-1特异度最高,而3项并联联合检测的灵敏度及串联联合检测的特异度、阳性预测值、正确率均高于各单项检测;受试者工作特征曲线(receiver operating characteristic curve,ROC)分析表明串联联合检测特异度及曲线下面积均最高。结论CK19、HBME-1和Gal-3可作为甲状腺乳头状微小癌的有效诊断指标,且3项指标串联联合检测可提高其准确率。Objective To investigate the expressions and diagnostic values of combined detecti on of cytokeratin 19(C K 19),mesothelioma(HBME-1)and galectin-3(Gal-3)in papillary thyroid microcarcinoma(PTMC).Methods Forty-eight patients with thyroid papillary microcarcinoma and forty-eight benign thyroid papillary lesions were selected from Hu’nan Prevention and Treatment Institute for Occupational Diseases from October 2021 to September 2022.Immunohistochemical methods were used to detect the expression of CK19,HBME-1 and Gal-3 in the two groups of lesions.Results The positive rates of CK19,HBME-1 and Gal-3 in PTMC group were significantly higher than those in benign thyroid lesions group,the differences were statistically significant(P<0.01).CK19 and Gal-3 had the highest sensitivity,and HBME-1 had the highest specificity.The sensitivity of three parallel combined detection and the specificity,positive predictive value and accuracy of three tandem combined detection were higher than those of each single detection.Receiver operating characteristic curve(ROC)analysis showed that the specificity and the area under the curve of tandem combination detection were the highest.Conclusion CK19,HBME-1 and Gal-3 can be used as effective diagnostic indicators for PTMC,and the combined detection of the three indicators can improve the accuracy.
