持续质量改进在降低跟骨牵引患者局部并发症中的应用效果分析

摘　　要：目的分析持续质量改进对下肢跟骨牵引患者局部并发症发生率的影响。方法选择福州市第二医院创伤骨科2021年1—12月跟骨牵引患者共80例随机分为观察组、对照组,各40例,分别实施常规护理、持续质量改进护理,对比两组患者局部并发症发生率。结果观察组跟骨牵引有效率90.0%(36/40)显著高于对照组72.5%(29/40),下肢深静脉血栓发生率5.0%(2/40)显著低于对照组22.5%(9/40),针眼渗血发生率35.0%(14/40)显著低于对照组60.0%(24/40),差异有统计学意义(P<0.05),两组患者足跟部压疮发生率比较差异无统计学意义(P>0.05),所有患者均未发生针眼感染。结论持续质量改进可有效降低下肢骨折患者跟骨牵引下肢深静脉血栓、针眼渗血的发生率,提高跟骨牵引的有效率。Objective To analyze the effect of continuous quality improvement on the incidence of local complications in patients with calcaneal traction.Methods A total of 80 patients with calcaneus traction in department of trauma and orthopedics of The Second Affiliated Hospital of Fuzhou from January to December 2021 were randomly divided into the observation group and the control group,with 40 cases in each group.Routine nursing and continuous quality improvement nursing were implemented,and the incidence of local complications was compared between the two groups.Results The effective rate of calcaneum traction in the observation group was 90.0%(36/40)significantly higher than that in the control group 72.5%(29/40),the incidence of lower extremity deep vein thrombosis was 5.0%(2/40)significantly lower than that in the control group 22.5%(9/40),and the incidence of needle eye bleeding was 35.0%(14/40)significantly lower than that in the control group 60.0%(24/40).The differences were statistically significant(P<0.05).There was no significant difference in the incidence of heel pressure ulcers between the two groups(P>0.05),and no needle eye infection occurred in all patients.Conclusion Continuous quality improvement can effectively reduce the incidence of lower extremity deep vein thrombosis and needle eye bleeding in patients with lower extremity fracture,and improve the effective rate of calcaneal traction.
