清筋术联合中医分期外治法治疗糖尿病足筋疽的临床疗效观察

摘　　要：目的 探讨清筋术联合中医分期外治法治疗糖尿病足筋疽的临床疗效。方法 选取2021年6—11月于上海中医药大学附属曙光医院住院治疗的78例糖尿病足筋疽患者,按照随机数字表法将患者分为试验组(采用清筋术联合中医分期外治法治疗)和对照组(采用常规外科治疗),每组39例。比较两组患者治疗2个月后的各项创面评分、治疗有效率和截肢(趾)率。结果 治疗2个月后,两组患者的各项创面症状评分均较本组治疗前降低,且试验组患者的各项创面症状评分均低于对照组患者,差异均有统计学意义(P<0.05)。试验组患者的治疗总有效率高于对照组患者,总截肢(趾)率低于对照组患者,差异均有统计学意义(P<0.05)。结论 清筋术联合中医分期外治法治疗糖尿病足筋疽具有较好的临床疗效,截肢(趾)率低。objective To explore the clinical efficacy of removing decaying tissues along the tendons in combination with staged and external treatment of traditional Chinese medicine(TCM) in diabetic foot tendon gangrene. Method A total of 78 patients with diabetic foot tendon gangrene who were treated in Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from June to November of 2021 were selected, the patients were randomly divided into experimental group(treated with removing decaying tissues along the tendons in combination with staged and external treatment of TCM) and control group(treated with conventional surgery treatment) by random number table method, with 39 cases in each group. The wound score of each item, therapeutic effectiveness rate and amputation(toe) rate were compared after 2 months of treatment. Result After 2 months of treatment, compared with that before treatment, the wound scores of two groups decreased, and the wound scores of patients in the experimental group were lower than those of the patients in the control group, the differences were statistically significant(P<0.05). The total therapeutic effectiveness rate of the experimental group was higher than that of the control group, the amputation(toe)rate in experimental group was lower than that in control group(P<0.05). Conclusion The treatment of removing decaying tissues along the tendons in combination with staged and external treatment of TCM in diabetic foot tendon gangrene has great clinical effect with lower amputation(toe) rate.
