热毒宁注射液联合抗生素治疗慢性阻塞性肺疾病急性加重期临床疗效研究

摘　　要：目的观察热毒宁注射液联合抗生素对慢性阻塞性肺疾病急性加重期(AECOPD)的治疗效果。方法将我院AECOPD患者70例随机分为对照组与治疗组各35例。对照组予头孢哌酮/舒巴坦注射液治疗,治疗组在对照组治疗基础予热毒宁注射液治疗,对比两组临床疗效,治疗前后的白细胞计数(WBC)、中性粒细胞百分比(NUE)、高敏C反应蛋白(hs-CRP)、降钙酶原(PCT)水平,各个中医症候评分,用力肺活量(FVC)、1s用力呼气容积(FEV1)、动脉二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))值。结果治疗后,治疗组总有效率显著高于对照组(P<0.05);两组WBC、NUE、hs-CRP、PCT水平,咳嗽、发热、痰黄稠、气短喘促、咳时引痛、口干欲饮、面赤、大便干燥评分,PaCO_(2)值与治疗前比较均明显下降(均P<0.05),治疗组WBC、NUE、hs-CRP、PCT水平,咳嗽、发热、痰黄稠、气短喘促、咳时引痛、口干欲饮、面赤、大便干燥评分,PaCO_(2)值均低于对照组(P<0.05);两组FVC、FEV1、PaO_(2)值较治疗前增高,治疗组FVC、FEV1、PaO_(2)值高于对照组(P<0.05)。结论热毒宁注射液联合抗生素治显著降低AECOPD患者炎症状态,改善中医症候评分,提高肺功能,减轻CO_(2)潴留,改善缺氧状态,疗效显著。Objective To observe the therapeutic effect of Reduning Injection combined with antibiotics on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Seventy patients with AECOPD were randomly divided into control group and treatment group,35 cases in each group.Control group was treated with cefoperazone/sulbactam injection,and treatment group was treated with Reduning Injection on the basis of control group.The efficacy of white blood cell count(WBC),neutrophil percentage(NUE),hypersensitive C-reactive protein(hs-CRP),procalcitonin(PCT)levels,various TCM symptom scores,forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),arterial carbon dioxide partial pressure(PaCO_(2)),arterial blood oxygen partial pressure(PaO_(2))value before and after treatment were compared between the two groups.Results After treatment,the total effective rate in treatment group was significantly higher than that in control group(P<0.05);the levels of WBC,NUE,hs-CRP,PCT,the scores of cough,fever,thick yellow phlegm,shortness of breath,shortness of breath,pain when coughing,thirsty,flushed face,and dry stool,PaCO_(2)value were significantly decreased compared with those before treatment(P<0.05),and were lower in treatment group than in control group(P<0.05);the values of FVC,FEV1 and PaO_(2)were higher than those before treatment in both groups,and were higher in control group than in control group(P<0.05).Conclusion Reduning Injection combined with antibiotic therapy can significantly reduce the inflammatory state of AECOPD patients,improve TCM symptom scores,improve lung function,reduce CO_(2)retention,and improve hypoxia state with significant curative effect.
